糖皮质激素治疗前列腺电切术后并发肾上腺皮质功能相对不全24例回顾性分析

摘　　要：目的：探讨糖皮质激素在前列腺电切术后并发肾上腺皮质功能相对不全的应用疗效。方法回顾性分析总结24例因良性前列腺增生行T U RP术，术后出现肾上腺皮质功能相对不全而应用氢化可的松治疗的临床经验。结果根据24例患者术后均出现不同程度淡漠、嗜睡、烦躁不安、胡言乱语等临床症状，检查血皮质醇水平正常或轻度下降，通过采取静脉滴注氢化可的松100～400 mg／d ，所有病例出现的相关临床症状均得到明显改善或治愈，无1例死亡，随访6～12个月均未复发。结论当老龄患者手术后出现肾上腺皮质激素相对不足症状时，及时应用糖皮质激素治疗安全有效且预后良好。Objective To study the efficacy of glucocorticoid in treating relative insufficiency of adrenocortical function complications after transurethral resection of the prostate （TURP） .Methods Clinical data of 24 cases of benign prostatic hyperplasia undertaken TURP were retrospectively analyzed .After surgery ,the clinical experience in treating relative insuffi‐ciency of adrenocortical function applying hydrocortisone was summarized .Results After surgery ,all patients showed var‐ying degrees of apathy ,drowsiness ,restlessness ,blathering and other clinical symptoms ,and their serum cortisol level was nor‐mal slightly declining .Hydrocortisone injection 100-400 mg/d was injected ,which apparently improved or cured relative clin‐ical symptoms .During the follow‐up of 6-12 months ,no recurrence was reported .Conclusions When relative insufficiency of adrenocortical function occurred to aged patients after surgery ,prompt application of glucocorticoid is safe and prognosis is fairly good .
