HIV感染患者进行高效抗逆转录病毒治疗的fMRI脑功能研究评价

摘　　要：目的研究评价HIV感染患者进行高效抗逆转录病毒治疗脑部功能区变化。方法采用Siemens 3.0 T磁共振成像系统进行受试者扫描。HIV感染患者45例,已行高效抗逆转录病毒治疗22例,未行治疗23例。实验任务为组块设计的双侧手部运动试验。通过SPM8软件对实验进行数据预处理及统计学分析,经过组内数据统计及组间数据比较分析,获取正常受试者和HIV感染者及治疗患者分组间脑部激活功能区差异。结果 HIV患者组中经抗病毒治疗患者左手运动任务激活区与未进行治疗患者间差异无统计学意义,治疗患者执行右手运动任务激活区包括左侧中央前回、中央后回、辅助运动皮层及右侧小脑均出现激活,未治疗患者在右侧颞极、海马及左侧小脑激活程度高于治疗后患者。结论高效抗逆转录病毒治疗有效抑制了部分脑区的异常激活,为治疗HIV侵入中枢神经系统及损伤机制提供新的方法。Objective To investigate the functional magnetic resonance imaging （fMRI） signal changes of HIV infected patients treated with highly active antiretroviral therapy （HAART）. Methods fMRI was performed in forty-five HIV infected patients. Of the 45 cases, 22 had received HAART （ study group） and 23 had not received HAART （ control group）. The task was block designed and related to the unilateral hand gripping. Group data statistics and comparative analysis among the groups were made, and the differences in brain activation function areas between the two groups were compared. Results No significant difference in the left hand motor task activated zone existed between the two groups. After HAART, patients of the study group showed activation in left precentral gyrus, postcentral gyrus, supplementary motor cortex and right cerebellum with right hand motor task. During the task, patients who had not received HAART showed increased activity in right temporal lobe, hippocampus and left cerebellum. Conclusion HAART can effectively suppress the abnormal activation area in the brain and this technique provides a new tool to investigate the mechanism of HIV-related cognition disorder.
