摘  要:目的应用优化的基于体素的形态测量学(VBM)技术,分析HIV阳性男性患者全脑灰质异常情况。方法选取47例HIV阳性男性受试者和与之年龄、性别匹配的23名HIV阴性者(正常对照组)进行磁共振3D T1WI扫描。利用基于统计参数图(SPM)8软件的VBM 8工具箱对扫描获得的图像进行预处理,对HIV患者和正常对照组的全脑灰质体积采用t检验进行比较,找出灰质体积差异区域的改变。结果应用VBM技术,发现患者组的左侧三角部额下回、左侧眶部额中回、左侧颞上回和双侧前扣带回较对照组减小(P<0.05)。中脑导水管周围灰质及侧脑室周围灰质较对照组增大。结论左侧三角部额下回、左侧眶部额中回、左侧颞上回和前扣带回灰质体积减小可能是评价HIV患者神经认知障碍的一项客观指标。Objective To investigate the related abnormalities of gray matter in male patients with HIV by using the optimized voxel-based morphometry (VBM) technique. Methods Forty-seven HIV-positive male subjects (study group) and 23 HIV-negative subjects with matched age and sex ( control group) were enrolled in this study. Three dimensional T1WI scanning was performed in all subjects. By using a version of VBM 8 in SPM 8, the images were pre-processed. The whole brain gray matter volume was determined, and the results were compared between the study group and the control group by using t-test. The difference in the gray matter volume was statistical analyzed. Results Compared to the control group, a significant decrease in gray matter volume was found in the left triangular part of the inferior frontal gyrus, left orbital middle frontal gyrus, left superior temporal gyrus and bilateral anterior cingulum in the study group ( P 〈 0.05 ), while an increases in gray matter volume was detected in periaqueductal gray and the gray around lateral ventricle in the study group. Conclusion The gray matter abnormalities determined by optimized voxel-based morphometry technique can be used to evaluate the neurocognitive impairment in HIV-positive patients.
