摘  要:目的:研究卵巢低反应患者抗苗勒管激素(AMH)和干细胞生长因子(SCF)的表达情况及二者的相关性。方法:选择年龄均小于35岁行体外受精-胚胎移植或卵细胞浆内单精子注射的卵巢低反应患者132例(卵巢低反应组)和正常对照组91例。采用ELISA检测两组患者血清和卵泡液中AMH蛋白和SCF蛋白的表达。分离并培养颗粒细胞,采用RT-PCR检测两组患者颗粒细胞中AMH mRNA和SCF mRNA表达相对灰度值,并对卵巢低反应患者颗粒细胞中两者表达进行相关性分析。结果:1与正常对照组比较,卵巢低反应组血清AMH蛋白表达(2.21±1.11 ng/ml vs 10.49±3.29 ng/ml)和卵泡液AMH蛋白表达(4.21±0.73 ng/ml vs 5.42±0.64 ng/ml)均显著降低,差异均有统计学意义(P<0.01;P<0.05)。2卵巢低反应组颗粒细胞中AMH mRNA表达相对灰度值较正常对照组显著降低(4.10±0.80 vs 6.20±1.40,P<0.01),而SCF mRNA表达相对灰度值较正常对照组明显增加(4.69±1.12 vs 3.10±1.40,P<0.05)。3卵巢低反应组颗粒细胞中AMH mRNA和SCF mRNA的表达呈显著负相关(r=-0.56,P<0.05)。结论:AMH的表达水平与卵巢反应性有关,卵巢低反应患者颗粒细胞中AMH表达显著降低,并与SCF的表达负相关,二者可能存在调控关系。Objective: To study the expressions of anti-mullerian hormone (AMH)and stem cell factor (SCF) and correlation between the two factors in patients with low ovarian response. Methoda:132 patients with low ovarian response ( low ovarian response group) and 91 control cases ( control group) all below 35 years old who experience in vitro fertilization and embryo transfer(IVF-ET) were recruited in the study, the level of AMH and SCF protein in serum and follicular fluid were examined by ELISA;the relative mRNA expression in granulose cells was detected by RT-PCR. The relationship between the expressions of AMH and SCF was analyzed by Pearson's correlation. Results:①Compared to control group,the expression of AMH had significantly decreased both in serum (2.21 ± 1.11 ng,/ml vs 10. 49 ± 3.29 ng/ml, P 〈 0. 01 )and follicular fluid(4.21 ± 0.73 ng/ml vs 5.42 ± 0.64 ng/ml, P 〈 0. 05) in study group. ②Compared to control group, the relative gray-scale value of AMH m RNA expression was decreased (4. 10 ± 0. 80 vs 6.20 ± 1.40, P 〈0. 01 ), but the relative gray-scale value of SCF mRNA expression was increased(4. 69 ± 1.12 vs 3. 10 ± 1.40, P 〈 0.05 ) in granulose cells in low ovarian response group. ③The expressions of AMH and SCF showed a significant negative correlation in granulose cells in the patients with low ovarian response( r = -0.56, P 〈0. 05). Conclusions:The expression of AMH is related with the ovarian response, which decreases in granulose cells of patients with low ovarian response, moreover AMH and SCF show a negative correlation which reveals AMH may regulate SCF's expression in human ranulose cells.
