宫腹腔镜下吸宫术治疗宫角妊娠32例临床分析

摘　　要：目的：探讨宫腹腔镜下吸宫术在宫角妊娠治疗中的应用价值。方法：回顾性分析雅安市第二人民医院2008年2月至2014年2月宫腹腔镜下吸宫术进行治疗的宫角妊娠32例患者的临床资料。结果：32例宫角妊娠患者中,11例有不同程度的宫腔粘连,2例为不全中隔子宫。32例患者中1例在吸宫术时宫角破裂,2例在宫腔电切镜刮除组织和电凝出血点时宫角破裂,立即在腹腔镜下行宫角破口修补术。32例患者术中出血10-200 ml,平均26±15 ml,宫角未破裂（29例）的出血10-30 ml,平均20±8 ml。手术时间10-24分钟,平均17±6分钟。32例血β-HCG术后45天均恢复正常,术后25-48天月经复潮。结论：宫腹腔镜下吸宫术是治疗宫角妊娠的一种安全有效的方法。Objective：To study the application value of diagnosis and treatment of cornual pregnancy by uterine aspiration under hysteroscopy combined with laparoscopy. Methods：The clinical data of 32 cases of cornual pregnancy who were treated by uterine aspiration under hysteroscope combined with laparoscope in our hospital from February 2008 to February 2014 were retrospectively analyzed. Results ：32 cases of cornual pregnancy patients were diagnosed by hysteroscopy, among which, 11 cases combined with intrauterine adhesions ,2 cases combined with incomplete separate uterus. 1 case and 2 cases underwent corneal rupture during uterine aspiration and tissue removing under hysteroscopy respectively, which received immediately repair under laparoscopy. All the 32 cases of cornual pregnancy were treated by uterine aspiration under hysteroscopy combined with laparoscopy successfuLLy ,the bleeding during operation ranged from 10 to 200 mL（ average 26 ± 15 ml）, for the cases without corneal rupture, bleeding ranged from 10 to 30 ml （average 20 ± 8 ml）. The operation time range from 10 to 24 minutes（average 17 ±6 minutes）. β-HCG level decrease to the normal level with in 45 days after the operation,the menstruation recovered within 25 ±48 days after operation. Conclusions. Uterine aspiration under hysteroscopy combined with laparoscopy is a safe and effective method to diagnose and treat cornual pregnancy.
