腭裂患儿全麻术后苏醒期的呼吸道管理策略

摘　　要：目的探讨腭裂患儿术后麻醉苏醒期呼吸道管理策略。方法选择在西安交通大学口腔医院进行腭裂修补手术患儿237例作为观察组,从物品准备、拔管前呼吸道管理、拔管指征掌握、拔管后呼吸道管理4个方面进行麻醉苏醒期呼吸道管理,并对之前用传统方法进行呼吸道管理的103例腭裂修补术患儿进行对照比较。结果观察组237例患儿中,治愈231例,占97.5%;平均住院9d,拔管后出现呼吸道并发症18例,占7.5%;其中喉痉挛6例,喉水肿11例,拔管后误吸1例,经过合理的治疗和护理,其并发症得到控制和缓解。对照组103例患儿中,治愈97例,占94.0%;平均住院11d,拔管后出现呼吸道并发症13例,占12.6%;其中喉痉挛5例,喉水肿7例,拔管后误吸1例,经过合理的治疗和护理,其并发症得到控制和缓解。两组患儿临床治愈率和平均住院日无显著性差异(P>0.05),但拔管后对照组出现呼吸道并发症显著高于观察组(P<0.05)。结论改进后的腭裂患儿术后麻醉苏醒期呼吸道管理策略明显减少了并发症的发生,为患儿平稳度过麻醉苏醒期奠定了良好的基础。Objective To discuss the management of postoperative respiratory tract at the children with cleft palate surgery. Methods 237 patients with Cleft palate repair were gathered as the observation group. The whole manage-ment included the preparation, respiratory management, extubation indications and respiratory tract management after extubation. 103 patients with traditional management were in the control group. Results 231 cases were healed in the observation group （97.5%）. The average length of stay was 9 days. 18 eases were with respiratory complications after extubation（7.5%）. The 6 cases were with laryngospasm, and other 11 cases were with I.aryngeal edema. One case was with aspiration. The complications were controlled after treatment and caring. 97 cases were healed in the control group （94%）. The average length of stay was 11 days. 13 cases were with respiratory complications after extubation （12.6%）. 5 cases were with laryngospasm, 7 cases were with Laryngeal edema. One case was with aspiration. The complications were also controlled after treatment and caring. There was no significant difference at the average length of stay between the two groups（P〉0.05）. But there was significant difference at the complications between the two groups（P〈0.05）. Conclusion The improved managements was good at reducing the complications and is useful for the child patients.
