情志与肠易激综合征证候的相关研究

摘　　要：目的:研究情志与肠易激综合征(IBS)证候、实验室指标间的关系,丰富中医情志和证候的研究。方法:对符合纳入标准的患者进行中医临床流行病学调查,获得忧、恐、怒、情绪低落、多疑、思、悲、惊、烦躁、焦虑、忧郁、紧张、抑郁等情志信息,检测患者5-羟色胺(5-HT)、神经肽Y(NPY)、血管紧张素Ⅱ受体(AT-Ⅱ)、同型半胱氨酸(HCY)、肿瘤坏死因子α(TNF-α)、金属硫蛋白(MT)等指标,运用SPSS 19.0进行统计分析。结果:忧、恐、怒、情绪低落、多疑等对证候的影响显著(P<0.01,P<0.05),而思、悲、惊、烦躁、焦虑、忧郁、紧张、抑郁等对证候无显著影响。相关分析中,AT-Ⅱ与焦虑,MT与怒、烦躁,TNF-α与紧张,HCY与烦躁均为显著正相关(P<0.01,P<0.05)。结论:忧、恐、怒、情绪低落、多疑对IBS中医证候的分布有显著影响,AT-II、MT、TNF-α、HCY和5-HT等含量的改变可能是情志诱发或加重IBS的生物学基础。Objective： Study on the correlation among emotions, TCM syndrome of irritable bowel syndrome, and laboratory indexes, to enrich the research of TCM emotions. Methods： Patients with inclusion criteria were investigated by TCM clinical epidemiology. The information of emotions such as melancholy, fear, anger, low spirit, suspicious, worrying, grief, terror, dysphoria, anxiety, melancholia, tension and depression were collected. And the indexes such as 5-HT, NPY, AT- Ⅱ, HCY, TNF- α and MT were detected. Then the correlation between the laboratory indexes and the emotions was analyzed by SPSS 19.0. Results： The emotions of melancholia, terror, anger, low spirit and suspicious had significant differences in TCM syndromes （P〈0.01, P〈0.05）, while there was no significant difference among the emotions of worry, grief, terror, dysphoria, anxiety, melancholia, tension and depression in TCM syndromes. The correlation analysis results showed that there were obvious correlations among AT- Ⅱ and anxiety; MT and anger and dysphoria; TNF- α and tension; HCY and dysphoria （P〈0.01, P〈0.05）. Conclusion： The emotions of melancholy, fear, anger, low spirit and suspicious could obviously influence the TCM syndrome of irritable bowel syndrome. The change of AT-Ⅱ, MT, TNF- α HCY and 5-HT may be the biological basis about emotions induce or aggravate IBS.
