通腑化瘀法应用于腹腔高压的临床研究

摘　　要：目的:观察通腑化瘀法对腹腔高压患者腹腔压力、呼吸功能的影响,并初步探讨其机制。方法:根据腹腔压力将入组的患者分为低IAP水平组和高IAP水平组,每组再分为对照组和通腑化瘀组两个亚组,每亚组10例患者。对照组给予腹腔高压的常规治疗,通腑化瘀组在对照组治疗的基础上加用通腑化瘀汤药,分别观察两大组内亚组之间腹腔压力、氧合指数、肠内营养开始时间、机械通气持续时间、ICU住院时间和各组28d死亡人数。结果:低IAP水平组患者应用通腑化瘀法后,腹腔压力降低,氧合指数增加,肠内营养开始时间提前,机械通气持续时间缩短(P<0.05)。高IAP水平组患者应用通腑化瘀法后,腹腔压力明显降低(P<0.05)。结论:通腑化瘀法可降低腹腔高压患者的腹腔压力,对于较低水平腹腔高压患者,可改善氧合,提前肠内营养开始时间,缩短机械通气时间和ICU住院时间。Objective： By observation of the effect of Tongfu Huayu method on intro-abdominal pressure （lAP） and respiratory function in patients with intra-abdominal hypertension （IAH） to investigate its mechanism. Methods： According to the IAP, 40 patients were divided into two groups： low lAP group and high IAP group, in which patients were randomly divided into the control subgroup and the Tongfu Huayu subgroup, with I0 patients in each subgroup. The control subgroup was received the routine treatment of IAH, while the Tongfu Huayu subgroup was received the Tongfu Huayu decoction based on the routine treatment of IAH. The lAP level, oxygenation index, beginning time of enteral nutrition, duration of mechanical ventilation, length of ICU stay and number of deaths were observed. Results： Compared with the control subgroup, the Tongfu Huayu subgroup in the low IAP group had obvious lower lAP level, higher oxygenation index, earlier enteral nutrition beginning time, and shorter mechanical ventilation duration （P〈0.05）. The Tongfu Huayu subgroup in the high lAP group had significant lower IAP level compared with the control subgroup （P〈0.05）. Conclusion： The Tongfu Huayu method could decrease the intro-abdominal pressure of patients with high IAP level. And Tongfu Huayu method could improve oxygenation index, shorten the enteral nutrition start time, mechanical ventilation duration and ICU stay time of patients with low IAP level.
