摘  要:目的探讨川崎病(KD)患儿血浆脑利钠肽(BNP)水平变化及其与心功能的相关性。方法选取KD患儿42例作为KD组,上呼吸道感染患儿30例作为对照组。采用酶联免疫法测定2组患儿的白细胞(WBC)、C反应蛋白(CRP)、红细胞沉降率(ESR)、血浆心肌肌钙蛋白(c Tn I)及血浆BNP。并采用多普勒超声心动图仪分别于急性期和恢复期获取KD患儿的心脏指数(CI)、舒张期二尖瓣血流频谱速度(E/A)、左室短轴缩短率(LVSF)和左室射血分数(LVEF)等参数。结果急性期KD患儿WBC、CRP、ESR、c Tn I及BNP明显高于对照组(P<0.05),WBC、CRP及BNP明显高于恢复期(P<0.05);而CI、LVSF及LVEF明显低于恢复期(P<0.05);血浆BNP与其CI、LVSF及LVEF均呈负相关(r值=-0.602、-0.564、-0.547、P<0.05)。结论急性期KD患儿血浆BNP会异常上升,恢复期逐渐下降,且患儿血浆BNP水平与心功能障碍存在正相关,检测血浆BNP水平对KD的及早诊断有指导意义。Objective To investigate the relationship between plasma brain natriuretic peptide( BNP)level changes in children with kawasaki disease(kD)and heart function. Methods 42 kD children were selected as kD group,and 30 children with upper respiratory tract infection were selected as control group. The two groups were measured by enzyme linked immunosorbent assay white blood cell( WBC),C reactive protein( CRP),erythrocyte sedimentation rate( ESR),plasma cardiac troponin( cTnI)and plasma BNP. And using Doppler echocardiography to check cardiac index( CI),diastolic mitral flow veloc-ity( E/A),left ventricular fractional shortening( LVSF)and ejection fraction( LVEF)in acute and recovery phase of the two group. Results The WBC,CRP,ESR,cTnI and BNP of the acute phase of kD were higher than those in the control group (P〈0. 05);The WBC,CRP and BNP of the acute phase of kD were higher than in recovery period(P〈0. 05),while CI, LVSF and LVEF of the acute phase of kD were lower than that in recovery period;There was inversely associated with plasma BNP and its CI,LVSF and LVEF. Conclusion The plasma BNP in kD acute phase will be abnormal rise,the plasma BNP in recovery period is decreased gradually. There is a positive correlation with the level of plasma BNP and heart dysfunction. Plas-ma BNP levels in the early diagnosis of kD has a guiding significance.
