自体腘绳肌腱单束重建前交叉韧带损伤15例

摘　　要：目的：探讨关节镜下自体腘绳肌腱单束重建前交叉韧带（ACL）损伤的临床疗效。方法：将经关节镜检查证实为ACL损伤15例患者,采用自体四股腘绳肌腱单束重建ACL,股骨端采用Endobutton固定,胫骨端采用界面螺钉固定。结果：所有患者术后均无严重并发症发生,均获得随访3~12个月。膝关节稳定性增强,Lachman试验阴性12例,弱阳性3例,末次随访时膝关节活动度达120°-160°,术前Lysholm膝关节功能评分56~76分,术后86~100分。所有患者均恢复伤前运动水平。结论：关节镜下自体腘绳肌腱单束重建ACL是治疗ACL损伤的有效方法。Objective：To investigate the clinical effects of autologous single-bundle hamstring tendon in the reconstruction of anterior cruciate ligament（ACL） under arthroscopy. Methods：Fifteen patients with ACL rupture diagnosed by arthroscopy were treated with the single bundle reconstruction of autologous quadrupled hamstring tendons. The femur and tibia ends were fixed by the Endobutton system and interference screw,respectively. Results：All patients were followed up for 3 to 12 months,no severe complication occurred,the knee joint stability was strengthened. The Lachman test was negative in 12 cases,and the Lachman test was weakly positive in 3cases. The range of knee joint activity was 120° to 160° at the last follow-up. The Lysholm scores of the knee joint function before and after operation were 56 to 76 and 86 to 100,respectively. The knee joint activities of all patients recovered. Conclusions：The reconstruction ACL with autologous single-bundle hamstring tendon under arthroscopy is a reliable method for recovering the stability of knee joint.
