摘 要:目的研究H型高血压与冠脉病变严重程度的关系,为冠心病的防治提供理论依据。方法对277例临床拟诊为冠心病的患者行冠状动脉造影,对冠心病的危险因素进行Logistic回归分析;再将患者分为H型高血压组、单纯高血压组、单纯高同型半胱氨酸血症组和对照组,比较各组Gensini积分。结果性别、高血压、糖尿病、高同型半胱氨酸血症、吸烟是冠心病的独立危险因素,其中高血压和高同型半胱氨酸血症存在交互作用。H型高血压组、单纯高血压组、单纯高同型半胱氨酸血症组Gensini积分均高于对照组,H型高血压组Gensini积分明显高于单纯高血压组和单纯高同型半胱氨酸血症组,差异均有统计学意义(P<0.05或0.01),单纯高血压组和单纯高同型半胱氨酸血症组Gensini积分差异无统计学意义(P>0.05)。结论高血压和高同型半胱氨酸血症对冠心病的发生有协同作用,H型高血压与冠脉病变严重程度密切相关。Objective To investigate the relationship between H-type hypertension and severity of coronary artery lesions,so as to provide theoretical basis for the prevention and treatment of coronary heart disease. Methods A total of 277 suspected cases with coronary heart disease underwent coronary angiography. The binary logistic regression analysis was used to analyze the risk factors of coronary heart disease. Patients were divided into H-type hypertension group,simple hypertension group,simple hyperhomocysteinemia group and control group. Then,difference of Gensini's score of the groups was compared. Results Binary Logistic regression showed that sex,hypertension,diabetes,hyperhomocysteinemia and smoking were independent risk factors for coronary heart disease. For the development of coronary heart disease,there was significant interaction between hypertension and hyperhomocysteinemia. Compared with that of the control group,the Gensini's scores of the H-type hypertension group,the simple hypertension group and the simple hyperhomocysteinemia group were all significantly higher. The Gensini's score of the H-type hypertension group was significantly higher than those of the simple hypertension group and the simple hyperhomocysteinemia group,so the difference was statistically significant( P〈0. 05 or 0. 01). And no significant difference was found in the Gensini's score of the simple hypertension group and that of the simple hyperhomocysteinemia group( P〉0. 05). Conclusion There is a synergistic action between hypertension and hyperhomocysteinemia on the development of coronary heart disease. H-type hypertension is closely related to the severity of coronary artery disease.
