参麦注射液对老年髋关节置换患者围术期血清S-100β蛋白的影响

摘　　要：目的观察手术麻醉过程中滴注参麦注射液对老年髋关节置换患者围术期血清S-100β蛋白的影响。方法选择60例ASAⅡ～Ⅲ级择期行髋关节置换术老年患者,分为两组各30例。A组于麻醉平稳后静脉滴注参麦注射液（1 ml/kg加至250 ml 0.9%氯化钠注射液）,手术结束前滴注完毕;B组只给予等量0.9%氯化钠注射液。于手术麻醉前一天（T0）,手术结束出麻醉恢复室（T1）,术后24小时（T2）和72小时（T3）分别抽取血样,测定血清S-100β蛋白浓度。结果两组各时间点血清S-100β蛋白浓度比较显示,T0时,A组与B组血清S-100β蛋白水平处于同一基线,浓度差异无统计学意义（P〉0.05）;T1、T2和T3三个时间点,A组血清S-100β蛋白浓度明显低于B组,差异有统计学意义（P〈0.01）。结论参麦注射液能有效降低老年髋关节置换患者术后血清S-100β蛋白浓度,对老年手术麻醉患者具有一定脑保护作用。Objective To observe the effect of Shenmai injection on perioperative serum S-100β protein in elderly patients during anesthesia for hip joint replacement surgery. Methods 60 elderly patients with ASA of gradeⅡ ～ Ⅲ undergoing hip replacement were randomly divided into Shenmai group（ group A） and control group（ group B）. Group A were given Shenmai injection（ 1 ml / kg Shenmai added to the 250 ml sodium chloride injection with an density of 0. 9%） by means of intravenous drip after the anesthesia was stable,and the drip would be finished before finishing the operation. However,group B were given the the same amount of sodium chloride injection with a density of 0. 9% only. Blood samples were respectively taken to test the protein S-100β level on the first day before operation anesthesia（ T0）,the time from the end of operation to leave the anesthesia recovery room（ T1）,24 hours after the operation（ T2） and 72 hours（ T3） after the operation. Results Comparison of serum S-100β protein levels at various time points showed that： serum S-100β protein levels of two groups at T0 were on the same baseline,so no significant difference was found（ P〉0. 05）; concentration of serum S-100β protein levels of group A at T1,T2 and T3were significantly lower than those of group B,so the difference was statistically significant（P〈0. 05）. Conclusion Shenmai injection can effectively decrease serum protein S-100β expression level of elderly patients with hip joint replacement,and it also provides certain cerebral protection for old patients undergoing anesthesia and operation.
