我院眼科2013年10月围术期抗菌药物预防性使用分析

摘　　要：目的：为眼科手术规范、合理使用抗菌药物提供参考。方法：采用住院病历回顾性调查方法,设计调查表格,调查我院眼科2013年10月1 220例Ⅰ、Ⅱ类切口手术患者全身及局部抗菌药物使用情况。结果：手术共1 220例次,全身预防性使用抗菌药物209例次,使用率为17.13%。其中Ⅰ类切口1 140例次,抗菌药物使用率11.67%;Ⅱ类切口80例次,抗菌药物使用率95.00%。全身预防性使用抗菌药物中,术前0.5~2 h给药占89.31%,≤24 h停药占33.08%。术前局部使用抗菌药物达96.07%,术中结膜下注射52例次,占4.26%。结论：我院眼科围术期抗菌药物不合理使用主要表现在术前0.5~2 h未给药、存在只在术后预防性使用抗菌药物、术后给药时间过长、术前术后频繁更改抗菌药物等方面有待规范。OBJECTIVE:To provide reference for the rational use of antimicrobial drugs in ophthalmic surgery. METHODS:By means of retrospective investigation of in-patient medical records,survey form was designed to investigate systemic and local antimicrobials use in 1 220 patients underwent type Ⅰand Ⅱ incision surgery in ophthalmology department of our hospital in Oct.2013. RESULTS:Among 1 220 cases,there were 209 cases of systemic prophylactic use of antimicrobial drugs with utilization rate of 17.13%. There were 1 140 cases of typeⅠ incision,and utilization rate of antimicrobial drugs was 11.67%;there were 80 cases of type Ⅱ incision,and utilization rate of antimicrobial drug was 95.00%. The systemic prophylactic use of antimicrobial drugs 0.5-2 h before operation accounted for 89.31%,and ≤24 h drug withdrawal accounted for 33.08%. The rate of preoperative local antimicrobials use was 96.07%. There were 52 cases of subconjunctival injection during operation,accounting for 4.26%.CONCLUSIONS:The perioperative irrational use of antimicrobial drugs in ophthalmology department of our hospital mainly include no medication 0.5-2 h before operation,postoperative prophylactic use of antimicrobial drugs,long postoperative medication duration and frequent replacement of antimicrobial drugs before and after operation,etc. Which should be standardized.
