芪苈强心胶囊改善心肌梗死后心力衰竭患者的临床症状

摘　　要：目的观察芪苈强心胶囊(黄芪、附子、人参、红花、泽泻、丹参、葶苈子、五加皮、陈皮、玉竹、桂枝)对于心肌梗死后心力衰竭患者的治疗效果。方法 148例心肌梗死后心力衰竭患者分为对照组和观察组各74例。对照组和观察组患者给予ACEI/ARB+β受体阻滞剂+安体舒通+阿托伐他汀+地高辛药物治疗,后者加服芪苈强心胶囊。观察两组患者治疗前后的心功能分级、心率、24 h尿量及6 min最大步行距离等指标变化。复查心脏彩超各项指标,检测hsCRP及N末端B型利钠肽前体(NT-pro BNP)水平变化,复查心电图观察PR间期及QT间期。结果经3个月治疗对照组和观察组有效率分别为78.4%和90.5%(P<0.05)。对照组和观察组的6 min步行距离,24 h尿量,NT-pro BNP,hs-CRP水平,心脏彩超LVED和LVEF,心电图PR间期等指标比较,后者改善显著(P<0.05);而心率和心电图QT间期两组未见显著差别(P>0.05)。结论芪苈强心胶囊能进一步改善心肌梗死后心力衰竭患者的临床症状。AIM To observe the therapeutic effect of Qili Cardiac Tonic Capsules（ Astragali Radix,Aconiti lateralis Radix praeparata,Ginseng Radix et Rhizoma,Carthami Flos,Alismatis Rhizoma,Salviae miltiorrhizae Radix et Rhizoma,Lepidii Semen,Descurainia Semen,Acanthopanacis Cortex,Citri reticulatae Pericarpium,Polygonati odorati Rhizoma,Cinnamomi Ramulus） on patients with heart failure after their myocardial infarction.METHODS One hundred and forty-eight cases of heart failure patients after their myocardial infarction were equally assigned into control group and observation group for ACEI / ARB ＋ β-blockers ＋ antisterone ＋ atorvastatin ＋digoxin administration,with the latter group dosed with Qili Cardiac Tonic Capsules. The clinic indexes taken into consideration to assess the two groups included cardiac function classification,heart rate,24 hours urinary output,6-min walking distance,indicators of cardiac ultrasound,the levels of hs-CRP and N terminal B-type natriuretic peptide precursor（ NT-pro BNP）,PR and QT intervals of electrocardiogram. RESULTS After a three-month treatment,the control group and the observation group reached 78. 4% and 90. 5% effective rate（ P 〈 0. 05）,respectively. All improvement in 6 min walking distance,24 hours urinary,NT-pro BNP,Hs-CRP level,LVED,LVEF,and PR interval were observed in both groups,and the latter showed to be even more significantly improved（ P 〈 0. 05）. But no significant difference lies in the heart rate and QT interval（ P 〉 0. 05）. CONCLUSION Qili Cardiac Tonic Capsules can obviously improve the therapeutic effect among the heart failures after their myocardial infarction.
