抗癌抑瘤方联合CapeOx方案治疗晚期结直肠癌

摘　　要：目的探讨抗癌抑瘤方（太子参,白术,茯苓,厚朴,白头翁等）联合Cape Ox方案（卡培他滨联合奥沙利铂）治疗晚期结直肠癌的协同增效作用。方法入选97例晚期结直肠癌患者随机方法分为对照组（48例）和研究组（49例）,两组接受Cape Ox化疗,后者加服抗癌抑瘤方。结果研究组49例晚期结直肠癌患者中完全缓解7例,部分缓解23例,总有效率61.2%;对照组48例患者中完全缓解3例,部分缓解15例,总有效率37.5%;两组差异有显著性（P〈0.05）。研究组和对照组患者中位疾病无进展生存时间分别为11.2个月（95%CI：8.9~13.5个月）、8.9个月（95%CI：8.3~9.5个月）,两组差异显著（P〈0.05）。治疗后研究组患者在生活质量评分、疼痛评分、部分不良反应及血清癌胚抗原、糖链抗原199、基质金属蛋白酶-7、基质金属蛋白酶组织抑制因子-2水平变化等方面占有优势,与对照组对比差异较显著（P〈0.05）。结论抗癌抑瘤方对Cape Ox方案治疗晚期结直肠癌具有协同增效作用,能够延长患者生存时间,改善患者生活质量,减轻部分不良反应。AIM To explore the synergistic effect and mechanism between the anti-tumor agent（ Pseudostellariae Radix,Atractylodis macrocephalae Rhizoma,Poria,Magnoliae officinalis Cortex,Pulsatillae Radix,etc.） and Cape Ox chemotherapy（ capecitabine plus oxaliplatin） in the treatment of advanced colorectal cancer. METHODS Ninty-seven patients with advanced colorectal cancer were randomly divided into control group（ n = 48） and trial group（ n = 49）. Patients of both groups were given Cape Ox therapy,but an anti-tumor agent was administered to the trial group. RESULTS In trial group（ n = 49）,the complete response number was 7 cases,the partial response number was 23 cases,the total effective rate was 61. 2%. In control group（ n = 48）,the complete response number was 3 cases,the parital response number was 15 cases,the total effective rate was 37. 5%,difference of two groups was prominet（ P 〈 0. 05）. The median PFS for the trial group and the control group were 11. 2 months（ 95% CI：8. 9-13. 5 months） and 8. 9 months（ 95% CI： 8. 3-9. 5 months）,respectively. After treatment,the advantages were found in the trial group in FACT,NRS,adverse events avoidance,and its levels of CEA,CA199,MMP-7,TIMP-2 were much lower,and statistical significance was observed（ P 〈 0. 05）. CONCLUSION The anti-tumor agent combined with Cape Ox chemotherapy can prolong the survival time,improve the quality of life and offset some adverse reactions,thus manage the advanced colorectal cancer effectively.
