黏膜表面麻醉联合鼻前庭浸润麻醉在儿童鼻出血等离子治疗中应用的回顾性分析

摘　　要：目的评价鼻腔黏膜表面麻醉联合鼻前庭浸润麻醉在等离子治疗儿童利特尔区鼻出血中对手术完成率、手术时间及术后3个月内复发率方面的影响。方法回顾性分析2010年9月至2013年9月于本院耳鼻喉科门诊治疗的147例诊断为鼻腔利特尔区出血儿童病历,根据麻醉方式的不同分为鼻腔黏膜表面麻醉+鼻前庭浸润麻醉组(A组)和单纯鼻腔黏膜表面麻醉组(B组),对两组手术完成率、完成手术者平均手术时间及术后3个月内复发情况进行评定。结果手术完成率:A组94.37%,B组65.79%,两组比较差异有统计学意义(P<0.05);完成手术者术后3个月内复发情况:A组4.50%,B组18.00%,两组比较差异有统计学意义(P<0.05);完成手术者平均手术时间:A组(5.14±2.62)mim,B组(15.32±5.93)min,两组比较差异有统计学意义(P<0.05)。结论鼻前庭浸润麻醉在儿童利特尔区出血等离子止血手术方面能够提高手术完成率,减少平均手术时间,降低术后3个月内出血复发率,值得临床推广及应用。Objective To evaluate the effects of the application about the nasal vestibule infiltration anesthesia combined with nasal mucosa surface anesthesia in the plasma hemostatic operation for the child Little area epistaxis.Methods 147 cases of children＇s medical records were retrospectively analyzed in E.N.T.department in our outpatient service from September,2010 to September,2013.All the medical records were divided into two groups（group A and group B）by different methods of anesthesia.There were 71 cases were tested by nasal vestibule infiltration anesthesia combined with nasal mucosa surface anesthesia in group A and 76 cases only were tested by the surface anesthesia in group B.The datum of the cases were collected and analyzed from three aspects：operation completion rates,average operation time and postoperative recurrence in 3 months from medical records.Results1）There were 67 cases finished the operation of plasma hemostatic in group A（71cases）,which accounted for94.37% and 50 cases finished in group B（76cases）,which accounted for 65.79%;2）There were 3cases recurred in3 months after operation in group A accounted for 4.50%and 9cases in group B accounted for 18.00%（P〈0.05）;3）The average operation time of the cases were（5.14±2.62）min in group A and（15.32±5.93）min in group B（P〈0.05）.Conclusions Nasal vestibule infiltration anesthesia used in the plasma hemostatic operation in the treatment of nasal bleeding of Little area of children can improve the completion rate and reduce the operation time and the recurrence rate in 3months of the operation,which is worthy of clinical popularization and application.
