凝血因子Ⅴ、凝血因子Ⅷ和组织因子与2型糖尿病合并冠心病的关系研究

摘　　要：目的探讨凝血因子Ⅴ、凝血因子Ⅷ和组织因子(TF)与2型糖尿病(T2DM)合并冠心病(CHD)的关系。方法选取我院2010年1月—2014年1月收治的T2DM患者34例,其中单纯性T2DM患者17例作为单纯T2DM组,T2DM合并CHD患者17例作为T2DM合并CHD组,冠心病患者中急性心肌梗死(AMI)者8例,心绞痛(UAP)者9例。观察两组患者TF、凝血因子Ⅴ促凝活性(FⅤ:C)、凝血因子Ⅷ促凝活性(FⅧ:C)、血管性血友病因子(v WF)和血小板聚集率(PAR),并分析其相关性。结果 T2DM合并CHD组患者FⅤ:C、FⅧ:C和TF水平高于单纯T2DM组,差异有统计学意义(P<0.05)。T2DM合并AMI组患者FⅤ:C、FⅧ:C和TF水平高于T2DM合并UAP组,差异有统计学意义(P<0.05)。FⅤ:C、FⅧ:C、TF与v WF、PAR均呈正相关。结论 FⅤ:C和FⅧ:C及TF水平可作为临床诊断T2DM患者并发CHD的有效指标,TF对T2DM合并CHD患者病情发展的判断有重要价值。Objective To study the relationship between coagulation factor Ⅴ,coagulation factor Ⅷ,tissue factor （TF）and type 2 diabetes mellitus（T2DM）complicated with coronary heart disease（CHD）. Methods A total of 34 T2DM patients were selected in our hospital from January 2010 to January 2014,thereinto 17 cases complicated with CHD（T2DM with CHD group）,17 cases did not（T2DM group）. In the 17 CHD cases,8 cases complicated with acute myocardial infarction （AMI）were served as T2DM with AMI group,9 cases complicated with unstable angina pectoris（UAP）served as T2DM with UAP group . TF,coagulant activity of coagulation factor Ⅴ（FⅤ：C）,coagulant activity of coagulation factor Ⅷ（ FⅧ：C）,von Willebrand factor（vWF）and platelet aggregation rate（PAR）of the two groups were observed,and the correlation analyzed. Results FⅤ：C,FⅧ：C and TF levels of T2DM with CHD group were higher than those of T2DM group,the differences were statistically significantly different（P 〈 0. 05）. FⅤ：C,FⅧ：C and TF levels of T2DM with AMI group were higher than those of T2DM with UAP group,the differences were statistically significantly different（P 〈 0. 05）. FⅤ：C,FⅧ：C,TF was positively correlated with vWF,PAR,respectively. Conclusion FⅤ：C,FⅧ：C and TF can be used as an effective index for diagnosising CHD in T2DM patients,TF has important value in judging the progression of T2DM patients complicated with CHD.
