摘 要:目的 测定糖耐量正常(NGT)人群和新诊断2型糖尿病患者血清Betatrophin、Irisin水平,探讨两者间的关系。方法 收集2012年12月—2013年9月在南方医科大学附属奉贤医院新诊断为2型糖尿病、年龄为40~60岁、体质指数(BMI)为18~28kg/m^2的患者(DM组)50例,同时选取与DM组年龄、性别和BMI相匹配的糖耐量正常(NGT)者(NGT组)50名,测定其血清糖脂代谢参数、Irisin和Betatrophin水平,计算稳态模型胰岛素分泌指数(HOMA-β)和稳态模型胰岛素抵抗指数(HOMA-IR)。结果 DM组的血清Betatrophin水平显著高于NGT组(P〈0.01)。DM组的血清Betatrophin水平与身高(r=0.439,P=0.001)、体重(r=0.463,P=0.001)、BMI(r=0.358,P=0.011)、腰围(r=0.341,P=0.016)、肌酐水平(r=0.344,P=0.014)呈正相关。NGT组的血清Betatrophin水平与年龄(r=0.343,P=0.016)、Irisin水平(r=0.346,P=0.014)呈正相关,与空腹胰岛素水平(r=-0.366,P=0.010)、HOMA-β(r=-0.361,P=0.010)呈负相关。胆固醇(β=-0.394)、三酰甘油(β=0.541)、餐后2h血糖(β=0.436)、HOMA-IR(β=-0.388)是血清Betatrophin的独立相关因素(P值均〈0.05)。结论 NGT人群血清Betatrophin水平明显低于新诊断2型糖尿病患者,且与Irisin呈正相关;而新诊断2型糖尿病患者的血清Betatrophin水平则与Irisin不相关,但与BMI、腰围、TC、TG、2h-BG和HOMA-IR相关,提示NGT人群而非新诊断2型糖尿病患者血清Betatrophin水平与Irisin存在一定关系。Objective To measure the levels of serum Betatrophin and Irisin in normal glucose tolerance (NGT) people and patients with newly diagnosed type 2 diabetes mellitus (T2DM), and to explore the relationship between Irisin and Betatrophin. Methods Fifty patients diagnosed as T2DM between December 2012 and September 2013, whose age ranged from 40 to 60 years and body mass index (BMI) ranged from 18 to 28 kg/m^2 , were enrolled in test group. Fifty age-, sex- and BMI-matched NGT people were allocated to control group. The parameters of serum glucose and lipid metabolism and the levels of serum Irisin and Betatrophin were determined. Insulin secretion index (HOMA-β) and insulin resistance index (HOMA-IR) in stable-state models were calculated. Results The level of Betatrophin in the test group was significantly higher than that in the control group (P〈0. 01). Serum Betatrophin level was positively correlated with body height ( r = 0. 439, P = 0.001 ), body weight (r=0.463, P=0.001), BMI (r=0.358, P=0.011), waist circumference (r=0.341, P=0.016) and creatinine level ( r = 0. 344, P = 0. 014) in T2DM patients. Serum Betatrophin level was positively correlated with age (r=0. 343, P = 0. 016) and Irisin (r = 0. 346, P = 0. 014) in normal controls, while it was negatively correlated with fasting insulin level (r = - 0. 366, P = 0. 010) and HOMA-β level ( r = - 0. 361, P = 0. 010).Multiple linear regression analysis showed that total cholesterol (β= -0. 394), triglyceride (β= 0.541 ), 2-hour blood glucose (β = 0. 436) and HOMA-IR (β = -0. 388) were independent factors related to serum Betatrophin (all P〈0. 05) . Conclusion Serum Betatrophin level in NGT is significantly lower than that in newly-diagnosed T2DM patients. Serum Betatrophin level is positively correlated to serum Irisin level in NGT, whereas there is no correlation between Betatrophin and Irisin in newly-diagnosed T2DM patients.
