彩色多普勒超声在2型糖尿病患者足背动脉病变中的应用价值

摘　　要：目的探讨彩色多普勒超声在2型糖尿病患者足背动脉病变中的应用价值。方法选择在我院就诊的2型糖尿病患者60例为观察组,另选择同期在我院就诊的非糖尿病患者60例为对照组。两组患者均在首次就诊时接受足背动脉彩色多普勒超声检查,比较并分析两组足背动脉血管内径、中层内膜厚度、粥样斑块及血流动力学差异。结果两组患者足背动脉的血管走形、管壁回声及内膜情况等比较,差异均有统计学意义(P<0.01),观察组足背动脉内膜中层厚度、粥样硬化斑块数量大于对照组(P<0.05或P<0.01),观察组管腔内径小于对照组(P<0.05),观察组足背动脉血流频谱可见双相波或三相波,Vs、Vd、Vr流速均较对照组减慢(P<0.05或P<0.01)。结论采用彩色多普勒超声检测可以在早期准确诊断2型糖尿病患者足背动脉病变,为临床的诊断及治疗提供依据,具有重要的临床应用价值。Objective To discuss application value of color doppler ultrasonography on acrotarsium arterial disease of patients with type 2 diabetes mellitus. Methods A total of 60 cases of patients with type 2 diabetes mellitus were selected, who were given the medical treatment in the hospital, and 60 cases of patients without diabetes mellitus in the same period were selected as control group. The patients in two groups were given color doppler ultrasonography on acrotarsium artery for the first visit, and the differences of vessel diameter, intima media thickness, atheromatous plaque and hemodynamics of acrotarsium artery were compared and analyzed. Results After comparison,vascular distribution, vessel wall echo, inner membrance and etc of acrotarsium artery of patients in two groups were showed significant difference（P〈0.01）. The intima media thickness and atheromatous plaque amount of acrotarsium artery of pa tients in observation group were much larger or more than those in control group（P〈0.05 or P〈0.01）. The inner diameter of patients in observation group was shorter than that in control group（P〈0.05）. The biphasic waveform or triphasic wave showed on artery frequency spectrum of acrotarsium artery of patients in observation group,and the Vs,Vd and Vr flow rates were slower than those in control group（P〈0.05 or P〈0.01）. Conclusion The detection color doppler ultrasonography can precisely diagnose acrotarsium arterial disease of patients with type 2 diabetes mellitus at early stage,and provide evidence for clinical diagnosis and treatment, which has significant clinical application value.
