不同鼻腔填塞材料在鼻内镜术后的应用研究

摘　　要：目的:探讨在慢性鼻窦炎鼻内镜手术后采用不同鼻腔填塞材料的应用效果,为提高该类患者的治疗质量提供指导依据。方法:对2012年1月至2013年11月收治的75例慢性鼻窦炎患者进行回顾性分析,均行鼻内镜手术治疗,按照不同的鼻腔填塞材料分成A、B、C三组,各组均25例,A组使用凡士林纱条,B组使用膨胀止血海绵,C组使用藻酸钙填塞物,对比三组患者的主观痛苦、鼻腔黏膜肿胀程度及症状复发状况。结果:C组患者手术后1d的睡眠失常、头痛及鼻塞鼻痛等的视觉模拟评分(VAS)、中度、重度鼻腔黏膜肿胀的患者比例均显著低于A组与B组患者,差异有统计学意义(P<0.05),而A组与B组比较,差异无统计学意义(P>0.05)。C组患者鼻腔黏连发生率、取出填塞物后出血率、鼻窦炎症迁延率显著低于A组,差异有统计学意义(P<0.05),而A组与B组比较,差异无统计学意义(P>0.05)。结论:在鼻内镜手术后应用不同鼻腔填塞材料的效果存在较大的差异,需要根据患者具体的鼻腔自然状态,选择合适的鼻腔填塞材料。Objective：To explore the application of different nasal cavity filling material in the chronic paranasal sinusitis after nasal endoscopic surgery .To provid guidance basis for improving the quality of the treatment of patients .Method：75 cases of patients with chronic sinusitis were retrospectively analyzed from January 2012 to November 2013 treated in our hospital , they were all given nasal endoscopic surgery , ac-cording to the different material of nasal cavity filling , they were divided into group A, B, C,25 cases in each group , Group A was used vaseline gauze , Group B was used hemostatic sponge , Group C was used calcium alginate stuffing , subjective pain , nasal mucosal swelling degree and the status of the recurrence of symptoms of patients of three groups were compared .Result：Sleep disorders of 1 day after surgery , head-aches and stuffy nose nasal pain visual analogue scale （ VAS） , moderate and severe nasal mucosal swelling ratio of patients of group C were significantly lower than those of patients of group A and B , differences were statistically significant （ P 〈0.05） , while the group A compared with group B , there was no statistically sig-nificant difference （ P 〉0.05） .Conclusion：There are many differences between the effect of the nasal cav-ity filling material applied after nasal endoscopic surgery , according to patients with specific nasal natural state, selecting the appropriate nasal cavity filling material .
