稳定及不稳定性心绞痛患者血清胱抑素水平变化及临床意义

摘　　要：【目的】探讨血清胱抑素水平与冠心病发病和病变程度的关系及在病情监测中的作用。【方法】选择冠心病患者142例,其中不稳定性心绞痛患者72例,稳定性心绞痛患者70例,并选造影正常者65例作为对照;测定血清胱抑素浓度,统计分析血清胱抑素与冠心病诊断、冠脉病变程度和危险分数间的关系。【结果】(1)血清胱抑素水平在稳定性心绞痛组和不稳定性心绞痛组均明显高于对照组,且不稳定性心绞痛组[(1.62±0.37)mg/L]明显高于稳定性心绞痛组[(1.07±0.38)mg/L](P<0.05)。(2)不稳定性心绞痛患者血清胱抑素与冠状动脉危险分数相关,相关系数r值为0.372(P<0.05)。(3)血清胱抑素水平随冠脉病变SYNTAX积分(www.syntaxscore.com下载)的增加,有逐渐增高的趋势。【结论】血清胱抑素水平可以反映冠状动脉病变程度,并可以作为冠心病病情监测的指标。【Objective】To investigate the relationship between Cystatin C level, diagnosis and severity of coronary heart disease as well as its role in monitoring patient＇s condition.【Methods】A total of 142 cases of coronary heart disease were selected for study, among which 72 were diagnosed as unstable angina pectoris（UAP） and 70 stable angina pectoris（SAP）, and 65＂healthy＂people were selected as control group who were identified as normal by coronary angiography. Serum Cystatin C levels were measured by immunoassay.Relationship between levels of Cystatin C and diagnosis, coronary lesion extent and jeopardy score was then analyzed by statistical methods.【Results】（1）Serum Cystatin C levels were significantly higher in patients with SAP or UAP than those in ＂healthy＂ people,and they were much higher in patients with UAP than those in patients with SAP（1.62±0.37 vs 1.07±0.38 mg/L, P〈0.05）.（2）In patients with UAP serum Cystatin C levels were correlated with coronary jeopardy scores（r = 0.47, P〈0.05）.（3）Serum Cystatin C levels intended to increase with coronary lesion SYNTAX score.【Conclusion】Cystatin C exists in initiation and development of coronary heart disease. It reflects coronary lesion extent and can be used as estimation of condition of coronary heart disease.
