摘　　要：目的：探讨臀上动脉穿支皮瓣修复骶尾部Ⅳ度褥疮的临床疗效。方法对2004年3月—2012年4月收治的19例骶尾部Ⅳ度褥疮患者，应用臀上动脉穿支皮瓣进行修复。结果19例臀上动脉穿支皮瓣中感染2例，边缘部分坏死3例，其余皮瓣均成活良好。术后平均随访23个月（3～39个月），皮瓣无臃肿，褥疮无复发，臀部外形满意。结论臀上动脉穿支皮瓣血供丰富，切取方便，操作简单，保留臀大肌功能，供区直接闭合，是修复骶尾部Ⅳ度褥疮的理想方法。Objective To investigate the clinical efficacy of superior gluteal artery perforator flap for repairing NPUAP stage Ⅳ sacrococcygeal pressure sores. Methods Nineteen cases of NPUAP stage Ⅳsacrococcygeal pressure sores from March 2004 to April 2012 were repaired with the superior gluteal artery perforator flaps. There were 15 males and 4 females with an average age of 54.7 years （range, 35～69 years）. Results Nineteen flaps survived successfully. The wounds infection in 2 cases; partial necrosis of flaps occurred in 3 cases. All patients were followed up for 23 months （range 3～39 months）, the appearance of gluteal region were satisfactory without bloated shape and no recurrence bedsores. Conclusion Due to the rich blood supply, easy to harvest and operate, function of the gluteus maximus was reserved and donor sites were closed primarily, the superior gluteal artery perforator flap can be used efficiently in reconstructions of stage Ⅳsacrococcygeal pressure sores.
