摘　　要：目的探讨经皮球囊扩张羟基磷灰石复合材料人工骨植骨治疗sanderⅡ、Ⅲ型跟骨骨折的价值。方法选取本院2008年1月—2012年1月收治的45例sanderⅡ、Ⅲ型跟骨骨折患者,其中22例采用自骨体植骨加克氏针内固定治疗(对照组),23例采用经皮球囊扩张羟基磷灰石复合材料人工骨植骨治疗(观察组),测量2组患者手术前,术后1 d、1年、2年的Bhler角与Gissane角变化,采用Maryland足部功能评分评价2组患者术后6个月、1年、2年的足部功能。结果 2组患者均获得随访,平均随访25个月,2组患者术后1d、1年、2年的Bhler角与Gissane角恢复情况比较差异无统计学意(P>0.05);术后6个月、1年、2年随访Mainland足部功能优良率比较差异无统计学意义(P>0.05)。结论经皮球囊扩张羟基磷灰石复合材料人工骨可用于填充sanderⅡ、Ⅲ型跟骨骨折并发骨缺损,可获得与自体骨相当的恢复疗效,值得临床推广。Objective To explore the value of percutaneous balloon dilatation combined with hydroxyapatite artificial bone grafting in treatment of sander Ⅱ, type Ⅲ fractures of the calcaneus. Methods 45 patients with sander Ⅱ, Ⅲ type fractures of the calcaneus were selected, and 22 of them were treated by bone grafting for treatment of bone from Kirschner wire internal fixation （control group）, 23 cases were treated by percutaneous balloon dilatation combined with hydroxyapatite artificial bone grafting （observation group）. Bohler angle and Gissane angle were measured before operation, Iday after surgery, 1 year, 2 years. The Maryland foot score evaluation was used to evaluate foot function after 6 months, 1 year, 2 years. Results The mean followup time was 25 months, and there were no significant differences of Bohler angle and Gissane angle recovery after 1 day, 1 year, 2 years between two groups （P〉0.05）. After 6 months, 1 year, 2 year follow-up, excellent rate of foot function showed no significant difference in two groups （P 〉 0.05）. Conclusion Pereutaneous balloon dilatation combined with hydroxyapatite artificial bone grafting can be used to treat the sanderⅡ, Ⅲtype calcaneal fractures, and it can obtain considerable recovery effect with autogenous bone, so it is worthy of clinical application.
