硝苯地平控释片和厄贝沙坦联合治疗青年原发性高血压的临床疗效及其对肾功能的影响

摘　　要：目的探讨硝苯地平控释片和厄贝沙坦联合治疗对青年原发性高血压的治疗效果及其对肾功能的影响。方法选取2012年1月到2014年1月就诊的青年原发性高血压患者共180例,随机分为三组,A、B、C三组各60例患者。A组患者采用硝苯地平控释片30 mg/次,1次/d;B组患者采用厄贝沙坦150 mg/次,1次/d;C组患者采用硝苯地平控释片和厄贝沙坦联合治疗法。观察三组患者治疗前后血压、肾功能指标等指标的变化情况,同时统计患者药物不良反应情况。结果三组治疗后血压均较治疗前明显下降(P<0.05),且C组降压效果明显优于A、B组,差异有统计学意义(P<0.05)。三组患者治疗后24 h尿蛋白、24 h尿白蛋白和尿β2-微球蛋白(β2-MG)均较治疗前明显下降(P<0.05),A组和B组上述指标比较,差异无统计学意义(P>0.05);C组患者上述指标变化效果明显优于A、B两组,差异有统计学意义(P<0.05)。三组患者治疗前后肌酐(Cr)、内生肌酐清除率(CCr)和尿素氮(BUN)变化不大,差异无统计学意义(P>0.05)。三组患者治疗初期均有不同程度的头晕、恶心、面色潮红等现象,但均可耐受。结论硝苯地平控释片和厄贝沙坦联合治疗青年原发性高血压在降压的同时,能减少尿蛋白,保护肾功能。Objective To investigate the effect of nifedipine and irbesartan combination therapy on renal function in young people with es-sential hypertension. Methods A total of 180 patients of young patients with essential hypertension form January 2011 to January 2014 were ran-domly divided into three groups,A,B,C three groups of 60 patients each. Group A was treated with nifedipine 30 mg;group B were treated with irbesartan 150 mg;group C were treated with nifedipine and Eritrea irbesartan combination therapy method. Observe changes in blood pressure,u-rine albumin,blood urea nitrogen（ BUN）and other indicators of the three groups before and after treatment,and adverse drug reactions in patients with statistics. Results After treatment blood pressure in the three groups were than significantly decreased before treatment（ P 〈0. 05）,and group C was better than group A and B. The difference was statistically significant（ P 〈0. 05）. Three groups of patients after 24 h urinary pro-tein,before 24 h urinary albumin and urinary β2-MG decreased significantly compared with treatment（ P 〈0. 05）,A group A and group B of the index,the difference was not statistically significant（ P 〉0. 05 ）;group C patients change these indicators is better than A,B groups,the difference was statistically significant（ P 〈0. 05）. Three groups of patients before and after treatment of Cr,BUN and CCr little change,the difference was not statistically significant（ P 〉 0. 05）. Early treatment of the three groups of patients have varying degrees of dizziness,nausea, flushing and other phenomena,but tolerable. Conclusion The treatment of nifedipine and irbesartan combined in young patients of essential hy-pertension can reduce blood pressureand reduce urinary protein and protect renal function at the same time.
