摘　　要：目的观察重组人脑利钠肽(rh BNP)治疗急性心肌梗死(AMI)伴心力衰竭患者的临床疗效及对血清肌酸激酶同工酶(CK-MB)、肌钙蛋白(Tn I)的影响。方法将68例AMI伴心力衰竭患者随机分为观察组38例和对照组30例,对照组给予常规治疗,观察组在此基础上加用rh BNP。观察2组临床疗效,治疗48 h后血清CK-MB、Tn I水平的变化及1周后心功能的变化。结果观察组总有效率(86.84%)显著高于对照组(63.33%)(P<0.05);治疗48 h后,2组血清CK-MB、Tn I水平均显著下降(P<0.01),且观察组显著低于对照组(P<0.01);治疗1周后,2组心功能均有不同程度的改善,且观察组(除左室收缩末期内径外)改善程度优于对照组(P<0.05或P<0.01)。结论 rh BNP可有效改善AMI伴心力衰竭患者的心功能,降低血清CK-MB、Tn I水平,且疗效显著。Objective To observe the efficacy of recombinant human brain natriuretic peptide （rhBNP） for treating patients with acute myocardial infarction （AMI） accompanied by heart failure and its effects on serum creatine kinase isoenzyme （CK-MB） and troponin （TnI）.Methods Sixty-eight patients with AMI accompanied by heart failure were randomly divided into observation group （n =38） and control group （n =30）.Both groups were given routine treatment,while the observation group was added rhBNP.Results The overall response rate of the observation group （86.84 %） was markedly higher than that of the control group （63.33 %） （P 〈 0.05） ; 48 h after treatment,serum CK-MB and TnI levels went down significantly in both groups （P 〈 0.01）,and those in the observation group were obviously lower than in the control group （P〈 0.01） ; 1 week after treatment,the heart function was improved differently in both groups,and the improved degree in the observation group （excluding left ventricular end systolic diameter） was superior to that in the control group （P 〈 0.05 or P 〈 0.01）.Conclusion For the patients with AMI accompanied by heart failure,rhBNP can effectively ameliorate their heart function and decrease the levels of serum CK-MB and TnI,with significant efficacy.
