摘 要:目的探讨加强ICU工作人员手卫生对控制ICU院内感染的临床意义。方法将2009年1月至2010年12月入住我院ICU 3 d以上并且作了院内感染相关检查的患者1374例定为A组。工作人员为患者进行各项操作,按照惯例进行洗手、擦手和手消毒。将2011年1月至2012年12月入住我院ICU 3 d以上并且作了院内感染相关检查的患者1591例定为B组。工作人员为患者进行各项操作,严格按照消毒技术规范进行洗手和手消毒,并监督落实,科室备好消毒擦手巾和床旁快速手消毒剂。两组患者在性别、年龄、病情严重程度、疾病种类和侵入性操作等多方面均无显著差异,具有可比性。对两组患者进行院内感染发生数、院内感染的病原体种类和住院时间进行比较。结果 B组院内感染的发生率为7.5%,明显低于A组的12.6%;B组院内感染的病原菌种类为7种,明显少于A组的15种;B组患者在ICU的平均住院时间明显短于A组。两组三者比较均具有显著差异性(P<0.05)。结论加强ICU工作人员手卫生,能显著减少ICU患者院内感染的发生,减少病原菌种类,缩短患者的住院时间,值得临床重视并推广应用。Objective To study the relationship between the implementation of the hospital staff's hand hygiene and the ICU nosocomial infection. Method Group A was the 1374 patients who were examined items related to nosocomial infection and stayed in ICU for more than three days from Jan.2009 to Dec.2010 ICU staffs nursed them with their own habits of washing hands and hands disinfection. Group B was the 1591 patients who were examined items related to nosocomial infection and stayed in ICU for more than three days from Jan. 2011 to Dec.2012 ICU staffs nursed them under supervision to wash and sterilized their hands strictly. ICU were supplied with bedside sterilized paper towel and rapid hand-disinfectant. There were no significant differences in age, sex, patient's conditions, sorts of diseases and invasive procedures. The incidence of cosocomial infection, the nosocomial infection pathogenic strains and the length of hospitalization were compared between group A and B. Result The incidence of cosocomial infection in group B was 7.5% and lower than that in group A(12.6%), the nosocomial infection pathogenic strains was 8 and less than that in group A(14), the mean length of hospitalization in ICU of group B and shorter than that in group A, which all had statistically difference(P〈 0.05).Conclusion Implement of ICU staff's hands hygiene could effectively control the incidence of nosocomial infection in ICU, reduced the strains of nosocomial infection and shortened the length of hospitalization significantly, so it is worthy of wide use clinically.
