2型糖尿病肾功能衰竭患者血液透析丙型肝炎病毒感染的相关因素分析

摘　　要：目的探究2型糖尿病肾功能衰竭患者在血液透析中发生丙型肝炎病毒(HCV)感染的相关因素,以降低HCV感染率。方法选取2011年1月-2014年5月于医院行维持性血液透析治疗的76例2型糖尿病慢性肾功能衰竭患者的临床资料进行回顾性分析,采用logisitic回归分析HCV感染的危险因素。结果 76例2型糖尿病慢性肾衰竭患者中发生HCV感染31例,感染率为40.79%;单因素分析显示,血液透析持续时间、每周血液透析次数、输血史、透析设备破膜、行透析治疗医院数5个因素与行血液透析治疗的2型糖尿病肾功能衰竭患者发生HCV感染有关(P<0.05);多元回归分析显示,血液透析持续时间(OR=2.010)、透析设备破膜(OR=2.131)以及行透析治疗医院数(OR=2.980)等均是2型糖尿病肾功能衰竭患者在血液透析中发生HCV感染的独立影响因素(P<0.05)。结论行维持性血液透析治疗的2型糖尿病慢性肾功能衰竭患者的HCV感染率较高,与血液透析持续时间、透析设备破膜及行透析治疗医院数等因素密切相关。OBJECTIVE To explore the related factors for the hepatitis C virus （HCV）infection in renal failure pa-tients with type 2 diabets undergoing hemodialysis so as to reduce the incidence of HCV infection.METHODS The clinical data of 7 6 type 2 diabets patients with chronic renal failure who underwent the maintenance hemodialysis from Jan 2011 to May 2014 were retrospectively analyzed,and the logistic regression analysis of the risk factors for the HCV infection was performed.RESULTS The HCV infection occurred in 31 of type 2 diabetes 76 patients with chronic renal failure,with the infection rate of 40.79%.The univariate analysis indicated that the duration of hemodialysis,frequency of hemodialysis in a week,blood transfusion history,rapture of membrane induced by di-alysis equipment,and number of hospitals where the hemodialysis was performed were the five related factors for the HCV infection in the type 2 diabetes patients with renal failure undergoing the hemodialysis （P〈0.05）.The multivariate regression analysis showed that the duration of hemodialysis （OR=2.010）,rapture of membrane in-duced by dialysis equipment （OR=2.131）,and number of hospitals where the hemodialysis was performed （OR=2.980）were the independent influencing factors for the HCV infection in the renal failure patients during the he-modialysis （P〈0.05）.CONCLUSION The incidence of HCV infection is relatively high in the type 2 diabetes pa-tients with chronic renal failure undergoing the hemodialysis,which is closely related to the duration of hemodialy-sis,rapture of membrane induced by dialysis equipment,and number of hospitals where the hemodialysis is per-formed.
