自体真皮结合负压封闭引流技术治疗小儿感染性创面研究

摘　　要：目的探讨自体真皮结合负压封闭引流技术治疗小儿感染性创面的临床疗效,指导临床合理治疗小儿感染性创面。方法回顾性分析88例小儿感染性创面患儿,将其分为观察组46例、对照组42例,观察组采用自体真皮结合负压封闭引流技术治疗,对照组采用自体真皮结合常规换药及清洁治疗,对比两组患儿植皮后的临床疗效、二次手术率、用药次数、治疗所需时间以及治疗期间的总费用、感染性创面的愈合率。结果治疗效果优良率观察组为91.3%、对照组为66.7%,观察组的治疗效果优良率明显高于对照组患儿,差异有统计学意义(P<0.05);患儿的二次手术率对照组为35.7%,观察组为13.0%,对照组明显高于观察组患儿,差异有统计学意义(P<0.05);观察组患儿的换药次数、平均治疗时间、平均治疗费用明显低于对照组,差异有统计学意义(P<0.05);观察组治疗1、2、3周后感染性创面愈合率均明显高于对照组,差异有统计学意义。结论自体真皮结合负压封闭引流技术治疗小儿感染性创面能提高治疗的优良率,减少二次手术率,降低患儿的用药次数、治疗时间以及治疗费用,改善患儿创面的愈合率,可作为临床常用的治疗手段。OBJECTIVE To explore the clinical effect of autologous dermis combined with vacuum sealing drainage on treatment of the children with wound infections so as to provide guidance for reasonable clinical treatment of the children with wound infections.METHODS A total of 88 children with wound infections were enrolled in the study and divided into the observation group with 46 cases and the control group with 42 cases;the observation group was treated with the autologous dermis combined with vacuum sealing drainage,while the control group was given the autologous dermis combined with conventional drug dressing and cleaning;the clinical efficacy after the skin grafting,rate of secondary surgery,frequency of drug administration,time of treatment,total cost of treatment, and healing rate of infectious wound were observed and compared between the two groups.RESULTS The excellent and good rate of curative effect was 91.3% in the observation group,significantly higher than 66.7% in the con-trol group （P〈0.05）.The rate of secondary surgery was 35.7% in the control group,significantly higher than 13.0% in the observation group （P〈0.05）.The frequency of drug dressing,mean treatment time,and mean treatment cost of the observation group were significantly lower than those of the control group（P〈0.05）.At 1, 2,and 3 weeks of treatment,the healing rates of infectious wound were significantly higher in the treatment group than in the control group（P〈0.05）.CONCLUSION The autologous dermis combined with vacuum sealing drainage can raise the excellent and good rate of treatment of the children with wound infections,decrease the rate of sec-ondary surgery,reduce the frequency of drug administration and treatment time as well as treatment cost,and im-prove the healing rate of wound,and it can be commonly used for the clinical treatment.
