集束化护理策略对ICU患者肠内营养相关性腹泻的影响

摘　　要：目的探讨集束化护理策略对预防肠内营养相关性腹泻的效果。方法以重症医学科肠内营养患者作为研究对象,其中2011年1—12月111例患者,采用传统护理方法作为对照组;2012年1月—2013年6月179例患者,采用集束化护理为观察组,比较二组肠内营养相关性腹泻发生率。结果对照组111例发生腹泻44例,发生率39.6%,观察组179例发生腹泻34例,发生率18.9%,二组比较肠内营养相关性腹泻发生率差异有统计学意义(P<0.01),且集束化护理组在机械通气时间及入住ICU的时间明显低于对照组(P<0.001)。结论采用有效的集束化护理,可以降低肠内营养相关性腹泻发生率,减少通气时间和入住时间。Objective Investigate the effect of bundle nursing strategies on preventing enteral nutrition - associated diarrhea for ICU patients. Methods We enrolled 179 patients from ICU of author＇s hospital. Those admitted patients since January 2012 to June 2013 were received bundle nursing, while 111 cases of conservative group enrolled patients hospitalized since January 2011 to December 2011. The incidence of enteral nutrition - associated diarrhea between th etwo groups was compared. Results Totally 44 cases di arrhea were in conservative group of 111 cases, and 34 cases diarrhea in trial group of 179 cases. The inci dence of enteral nutrition- associated diarrhea was 39.5% in conservative group and 18.9% in trial group （39.5% vs 18.9% , P〈0.05）. Duration of mechanical ventilation and ICU stay was lower in all patients from the group of bundle nursing strategies than that in the normal controls. Conclusion Effective bundle nursing strategies can reduce the incidence of enteral nutrition- associated diarrhea, the duration of mechan ical ventilation and ICU stay.
