多囊卵巢综合征3种常见中医证型卵巢超声形态学参数对比分析

摘　　要：目的探讨多囊卵巢综合征3种常见中医证型与卵巢超声形态学参数之间的联系,进而为中医辨证提供客观依据。方法选择中医辨证为肾虚型、痰湿阻滞型、气滞血瘀型的多囊卵巢综合征病例,测量各证型患者的卵巢超声形态学参数,分析不同证型之间卵巢超声形态学参数的差异。结果 1共纳入178例病例,其中痰湿阻滞型71例(39.9%),肾虚型55例(30.9%),气滞血瘀型52例(29.2%)。2痰湿阻滞型患者的卵泡数目(FN)、卵巢体积(V)与肾虚型、气滞血瘀型的差异有统计学意义(P<0.05);各型间质面积/总面积(SA/TA)差异无统计学意义(P>0.05)。结论不同证型的多囊卵巢综合征患者在卵巢超声测量指标上存在差异,其中痰湿阻滞型有更为明显增大的卵巢体积和增多的卵泡数目,可以为中医辨证分型提供客观标准。Objective Through the ultrasonic parameters of different TCM syndrome types in polycystie ovary syndrome, this paper discussed the possible relatinnship between parameters and types of TCM syndromes, and provided a scientific basis for the syndrome dift）rentiation of traditional Chinese medicine treatment. Methods Patients with polycystic ovarian syndrome of the kidney deficiency type, phlegm- dampness stagnation type and qi stagnation and blood slasis type were recruited, and their ultrasonic parameters were measured. The differences of ultrasonic parameters were analyzed. Results （1）One hundred and seventy-eight cases were recruited, 71 cases were phlegm-dampness stagnation type （39.9%） , 55 cases were kidney deficiency type （30.9%） and 52 cases were qi stagnation and blood stasis type （29.2%）. （2） The follicle number （FN） and ovary volume of the phlegm-dampness stagnation type were significant different from those of the kidney deficiency type and qi stagnation and blood stasis type （P 〈 0.05 ） ; no significant difference was found in SA/TA among the three types （ P 〉 0.05 ）. Conclusion Different syndrome types of polycystic ovarian patients in the study on the ultrasonic measurement are different. The phlegm-dampness stagnation type is superior to the other in increasing the ovarian volume and the follicle number. This research has provided an objective of the standard of TCM patterns.
