摘 要:目的研究复方水牛角颗粒治疗急性痛风性关节炎的作用机制。方法 Wistar大鼠40只,随机分为正常组、模型组、中药组、西药组。大鼠单侧踝关节内一次性注入尿酸钠盐溶液诱导急性痛风性关节炎模型,正常组注射等量0.9%Na Cl溶液。造模后4 h,中药组和西药组分别给予复方水牛角颗粒、秋水仙碱灌胃,正常组和模型组灌胃等量0.9%Na Cl溶液。每日1次,共3 d,72 h后取大鼠血清、关节滑膜组织,测量大鼠踝关节周径,运用凝胶阻滞实验观察NF-κB活性;蛋白印迹法、荧光定量PCR法检测TNF-α、IL-8的蛋白和基因表达。结果与正常组比较,模型组大鼠踝关节肿胀程度明显增加,滑膜组织中NF-κB活性、TNF-α、IL-8蛋白和基因表达显著升高(P<0.01)。与模型组比较,中药复方水牛角颗粒可明显减轻模型大鼠踝关节肿胀程度,抑制NF-κB活性以及TNF-α、IL-8的蛋白和基因表达(P<0.05,P<0.01)。结论复方水牛角颗粒具有良好的抗急性痛风性关节炎作用,其部分作用机制可能为抑制NF-κB活性、下调TNF-α、IL-8表达,以减轻组织的炎性损伤。Objective To observe the mechanism of " Compound Buffalo Horus Granules" in treating acute gouty arthritis. Methods Forty Wister rats were randomized into normal group, model group, Chinese medicine group and western medicine group. Model of acute gouty arthritis was induced with the injection of MSU into rats' ankle joint cavity. Normal group was injected with the same amount of 0.9% NaCI solution. Four hours after injection, buffalo horus group, colchicine group were given cm'responding medicines for 3 days, while 0. 9% NaCl was used to normal group and model group, once a day, with the course of 3 days. Rats were executed to take serum and ankle synovial tissue, and measured the ankle circumference. The activity of NF- kappa B was measured by electrophoretic mobility shift assay ( EMSA ). Cytokine protein and gene expressions including TNF-α, IL- 8 in arthritic synovium of rats were determined respectively by Western blot assay, reverse transcription polymerase chain reaction (RT-PCR). Results Compared with the normal group, the ankle swelling, activity of NF- kappa B, TNF-alevels, and expression of [L-8 in synovial tissue of the model group were increased markedly ( P 〈0.01 ). Compared with the model group, the ankle swelling was alleviated, activity of NF-kappa B, the expressions of TNF-α and IL-8 were inhibited in the Chinese medicine group after treated with "Compound Buffalo Horus Granules" (P 〈0.05, P 〈0. 01 ). Conclusion "Compound Buffalo Horus Granules" is effective to treat acute gouty arthritis in rats. Its mechanism may be associated with the inhibitory effect on NF-kappa B activity and down-regulatory effect on TNF-α and IL-8 expression.
