摘　　要：目的：探讨后路经关节螺钉内固定治疗创伤性寰枢椎不稳定疗效。方法采用以枢椎下关节突下缘中点为进钉点的后路改良节螺钉内固定治疗寰枢椎不稳定46例。结果术后随访5～48个月，平均21个月。寰枢椎获解剖复位44例，未解剖复位2例，内固定位置均良好。寰枢椎于术后2～3个月均获得骨性融合，临床症状缓解，无并发症发生。结论后路改良经关节螺钉内固定术，操作简便，疗效可靠，可作为治疗寰枢椎不稳定的有效术式。Objective To explore the effects on posterior transarticular screw fixation for traumat-ic atlantoaxial instability.Methods A total of 46 cases of atlantoaxial instability underwent modified pos-terior transarticular screw fixation through the central point of lower margin of the axial inferior articular process.Results All cases were followed up for 5 to 48 months（21 months on average）.Anatomical res-toration was achieved in 44 out of the 46 cases,and the position of internal fixation was satisfactory.Atlan-toaxial bony fusion was obtained within two to three months.The clinical symptoms were relieved and no complications occurred.Conclusion Modified posterior transarticular screw fixation is recommended as an effective treatment for atlantoaxial instability due to its simple procedure and satisfying curative effect.
