单双极钩形钳与国产超能剪在腹腔镜下结直肠癌根治术中的应用效果

摘　　要：目的比较单双极钩形钳与国产超能剪在完全腹腔镜下乙状结肠/直肠癌根治术中的临床应用效果。方法在完全腹腔镜下行乙状结肠癌/直肠癌根治术患者126例,其中采用单双极钩形钳进行手术者60例(钩形钳组),采用国产超能剪进行手术66例(超能剪组)。对比两组手术时间、术中出血、术后肛门排气时间及并发症情况。结果两组患者均顺利完成手术。超能剪组手术时间、术中出血量均明显少于钩形钳组(P<0.05);两组术后出血、吻合口漏、术后感染等并发症发生率及术后肛门排气时间比较,差异均无统计学意义(P均>0.05)。结论单双极钩形钳与国产超能剪应用于完全腹腔镜下乙状结肠/直肠癌根治术均顺利完成手术,达到治疗效果,但国产超能剪在功能上优于单双极钩形钳。Objective To explore the effects of homopolar-dipolar hooked dissector and domestic bipolar coagulation scissors applied to total laparoscopic radical resection for sigmoid / rectal carcinoma. Methods Of 126 patients underwent total laparoscopic radical resection for sigmoid / rectal carcinoma,60 received surgery using subtotal homopolar-dipolar hooked dissector（ hooked dissector group） and 66 received surgery using domestic bipolar coagulation scissors（ coagulation scissors group）. The operation duration,intraoperative blood loss,postoperative anal exhaust time and incidence of complications were compared between two groups. Results The patients in two groups underwent operations successfully.The operation duration,intraoperative blood loss in the coagulation scissors group were significantly less than those in the hooked dissector group（ P〈0. 05）. There was no significant difference in the incidence of complications（ postoperative bleeding,anastomotic leakage and postoperative infection） and postoperative anal exhaust time between two groups（ all P〉0. 05）.Conclusion Both homopolar-dipolar hooked dissector and domestic bipolar coagulation scissors applied to total laparoscopic radical resection for sigmoid / rectal carcinoma achieve the successful operation,with satisfactory effects. Domestic bipolar coagulation scissors is superior to homopolar-dipolar hooked dissector regarding the function.
