2型糖尿病患者外周血白细胞及其相关因素分析

摘　　要：目的探讨2型糖尿病患者外周血白细胞计数及其相关影响因素。方法采用随机整群抽样方法,对崇明县城桥镇40～70岁2 270名常住居民进行调查,内容包括体格检查、75 g葡萄糖口服耐量试验、血常规和血脂等生化检查。结果 2 270名居民检出2型糖尿病655例,无糖尿病1 615例。单因素分析显示2型糖尿病组的血压、体重指数（BMI）、白细胞计数（WBC）、中性粒细胞计数（NEUT）、总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL-C）明显高于无糖尿病组（P〈0.05）,而高密度脂蛋白胆固醇（HDL-C）则明显低于无糖尿病组（P〈0.01）。多元回归分析显示WBC、BMI、餐后2 h血糖（2h PG）、TG、TC、NEUT是2型糖尿病的危险因素。结论 2型糖尿病患者可能存在慢性炎症状态,炎症反应在糖尿病的发生发展中起到重要作用。Objective To investigate peripheral white blood cell（ WBC） count and its related influencing factors among patients with type 2 dibetes mellitus（ T2DM）. Methods Random cluster sampling was adopted to conduct a survey among 2270 residents aged from 40 to 70 years in Chengqiao Town,Chongming County. The items of questionnaire included physical examination,75 g oral glucose tolerance test,and biochemical examination including routine blood test and serum lipid. Results Among 2270 residents,655 with T2 DM and 1615 without diabetes mellitus. The result of univariate analysis showed that blood pressure,body mass index（ BMI）,WBC,neutrophil（ NEUT）,total cholesterol（ TC）,triglyceride（ TG） and low density lipoprotein cholesterol（ LDL-C） in T2 DM residents were higher than those in the residents without diahetes mellitus（ P〈0. 05）,while high density lipoprotein cholesterol（ HDL-C） in T2 DM residents was significantly lower than that in the residents without diahetes mellitus（ P〈0. 001）. The result of multiple regression analysis showed that WBC,BMI,2-hour plasma glucose（ 2h PG）,TG,TC and NEUT were risk factors for T2 DM. Conclusion Patients with T2 DM might suffer from chronic inflammation,and inflammatory reaction plays an important role in the occurrence and development of diabetes.
