中西医结合治疗胆道术后综合征的疗效

摘　　要：目的探讨中西医结合治疗胆道术后综合征的临床疗效。方法对32例胆道术后综合征患者均采用阿莫西林胶囊0.5 g口服,3次·d-1;左氧氟沙星片0.5 g口服,2次·d-1;甲硝唑片0.4 g口服,3次·d-1;维生素C片0.2 g口服,3次·d-1;门冬氨酸钾镁片0.149 g×片,口服,3次·d-1,并根据血常规及胆道炎症情况及时停用抗生素。同时,采用柴黄六金汤加减治疗。观察32例患者治疗前、治疗14 d后肝功能指标[谷丙转氨酶(ALT)、碱性磷酸酶(ALP)、谷氨酰转移酶(GGT)和总胆红素(TBIL)]及肝外、肝内胆管直径和胆总管壁厚情况。结果与治疗前比较,32例患者治疗14 d后ALT、ALP、GGT和TBIL水平均明显降低(均P<0.05);肝外、肝内胆管直径和胆总管壁厚亦明显减小(均P<0.05)。结论中西医结合是治疗胆道术后综合征的一个相对理想的方案。Objective To explore the clinical efficacy of combined traditional Chinese and Western medicine in the treatment of biliary postoperative syndrome. Methods Thirty-two patients with biliary postoperative syndrome were orally treated with amoxicillin capsules（0.5 g,3 times per day）, levofloxacin hydrochloride tablets（0.5 g,twice per day）,metronidazole tablets （0.4 g,3 times per day）, vitamin C tablets （0.2 g, 3 times per day）,potassium magnesium aspartate tablets （0.149 g×4 tablets, 3 times per day） and Chaihuang Six soup. In addition, antibiotics were used or withdrawn according to blood routine test and biliary tract inflammation situation.The levels of alanine transaminase（ALT）,alkaline phosph-atase（ALP）, gamma-glutamyltransferase （GGT） and total bilirubin （TBIL）, diameters of intrahepatic and extrahepatic bile ducts and thickness of common bile duct were measured before and after treatment for 14 days. Results After treatment for 14 days, the levels of ALT, ALP, GGT and TBIL were significantly reduced and the diameters of intrahepatic and extrahepatic bile ducts and thickness of common bile duct were obviously improved in all patients （ P〈0 . 05 ） . Conclusion Combined traditional Chinese and Western medicine is ideal for treating biliary postoperative syndrome.
