摘  要:目的了解男-男性行为者(MSM)买性商业性行为(商业性行为)影响因素。方法对绵阳市建立的MSM人群开放性队列进行随访调查,采用滚雪球采样法招募人员,进行相关行为学调查和血清学检测。结果调查男-男商业性行为的398例中,5.8%有过商业性行为,3.4%近6月有商业性行为,每次使用安全套91.7%。有或无商业性行为者,HIV确认感染率分别为17.4%和5.3%、HIV新发感染率分别为10.6/100人年和3.5/100人年、梅毒现症感染率分别为17.4%和8.8%(P均>0.05)。单因素分析年龄、职业、婚姻状况、近1年接受过艾滋病咨询检测、自认HIV感染风险、认识当地MSM朋友数、近6月家庭型寻找性伴、性伴最小年龄、男性肛交数、群交、有过异性性行为、固定性伴侣(BF)期间多性伴、习惯携带安全套、出柜、累计BF数、累计偶然性伴数、性行为安全套破损和吸毒与商业性行为相关(P均<0.05),多因素分析商业性行为的独立影响因素包括年龄、认识当地MSM朋友数、出柜、吸毒、自认HIV感染风险、近6月男性肛交数、近6月群交和有过异性性行为(P均<0.05)。结论 MSM买性商业性行为高危行为突出、多样,是HIV感染和传播的核心亚群,应针对其行为特征采取干预措施。Objective To explore the the associated factors of commercial sexual behavior among men who have sex with men (MSM). Methods Follow-up investigation was conducted in the MSM open queues, and snowball sampling method was adopted to recruit the subjects. Behavioral survey and serological test were both carried out for determine the associated factors. Result In the 398 cases of MSM, 5. 8% experienced commercial sex, 3.4% experienced commercial sex in the recent six months, of which 91.7% used condom for each time. Confirmed infection rate of HIV for MSM who had/hadn' t commercial sex was respectively 17.4% and 5.3%; the new infection rate was 10. 6/ 100 persons per year and 3.5/100 persons per year,and the current infection rate of syphilis were respectively 17.4% and 8.8% ( all P 〉 0.05 ). Single factor analysis showed that age, occupation, marital status, AIDS counseling examination within a year, selfdeemed risk of HIV infection, number of local MSM friends, sexual partner of family type within the last six months, the smallest age of sexual partners, times of anal sex with male, group sex, heterosexuality, multi sexual partners during stable affection, habit of carrying condom, disclosure, number of stable male sexual partners, times of one night stand, condom damage , drug taking, and factors were related to commercial sex ( all P 〈 0.05 ) ; multi - factor analysis revealed that the associated factors included age, number of local MSM friends, disclosure, drag taking, self- deemed risk of HIV infection, anal sex with male in last six months, and group sex in last six months ( all P 〈 0.05). Conclusion The high risk associated factors of MSM commer- cial sex are salient and various and this population is the core sub - group for infection and spreading HIV and thus deserve more targeted counter measures.
