摘　　要：目的了解北京市海淀区新生儿脐带血麻疹抗体水平,为控制小月龄儿童麻疹发病提供科学依据。方法随机选取在北京市海淀区妇幼保健院分娩的产妇及其新生儿,采用酶联免疫吸附试验方法(ELISA)检测脐带血麻疹Ig G抗体。结果共采集1 027份脐带血标本,脐带血麻疹Ig G抗体阳性率为91.24%,达到保护性水平为54.63%,几何平均浓度(GMC)为897.113 m IU/ml。本市和外来户籍母亲新生儿脐带血麻疹抗体GMC分别为787.07 m IU/ml和967.88 m IU/ml,阳性率分别为87.76%和93.31%,不同户籍母亲之间抗体水平和阳性率差异有统计学意义。不同性别、胎龄和体重新生儿脐带血麻疹抗体GMC和阳性率差异无统计学意义。结论北京市海淀区新生儿麻疹抗体达到保护性水平较低,母亲为本市户籍的低于外地户籍,可以通过育龄期妇女接种麻疹疫苗来提高,以达到预防小月龄婴儿麻疹发病的目的。Objective To explore the level of measles antibody in neonatal umbilical cord blood in Haidian district of Beijing, and provide scientific basis for controlling the onset of measles in infants. Methods The pregnant women in Haidian Maternal and Child Health Hospital and their newborns were randomly collected. The neonatal umbilical cord blood was tested for the level of measles IgG antibody by the means of enzyme - linked immunosorbent assay （ELISA）. Results Among 1027 neonates, the positive rate, protection rate and geometric mean concentration （GMC） of measles IgG antibody were 91.24%, 54. 63% and 897. 113 mIU/ml, respec- tively. The GMC of measles antibody in neonatal umbilical cord blood from native mothers and floating mothers were respectively 787.07 mIU/ml and 67.88 mIU/ml, and accordingly the positive rates were 87.76% and 93.31%. ~ The level of measles antibody and the positive rate of neonatal umbilical cord blood significantly cor- related with the mothers＇ census register. The GMC of measles antibody and positive rate showed no statistics difference in umbilical cord blood from weight. Conclusion The protection rate neonates in terms of neonatal gender, gestational age, and neonatal of measles IgG in neonatal is relatively low in Haidian district of Beijing. Native neonatal antibody levels are lower than those floating population. The effective way to prevent onset of measles in neonates is to carry out the strategies on measles immunization for women at childbearing age.
