摘 要:目的分析2003~2013年间的碘缺乏病调查结果,了解儿童碘缺乏病病情及碘营养状况,评价碘缺乏病防治效果。方法采用人口容量概率比例抽样(PPS)方法,随机抽取30所小学调查点,每个调查点随机抽取8~10周岁学生40~50名(男女各半)作为调查对象;采用触诊法检查甲状腺肿大情况和B超法测量甲状腺容积;采集居民户盐样、学生家中盐样和尿样以及本地居民和学生饮用水样进行碘含量检测;在每个调查点抽取五年级30名学生和该村5户家庭妇女同时进行健康教育知识问卷调查。结果于2003、2005、2009和2013年共调查8~10周岁小学生5 174人,甲状腺肿大率(触诊法)分别从2003年的3.26%下降到2013年的1.00%,;合格碘盐食用率在89.1%~98.1%之间,盐碘均数由2003年的34.5 mg/kg缓慢下降为2013年的25.3mg/kg;小学生尿碘中位数在218.4μg/L^290.8μg/L之间,均低于300μg/L;尿碘50μg/L以下的比例均小于0.6%,小学生碘缺乏病知晓率由2003年的96.0%逐年下降到2013年86.9%,家庭妇女健康教育知晓率相对平稳,在90%之间波动。结论南通市已达到持续消除碘缺乏病阶段目标,学龄儿童人群碘营养水平更加趋向理想水平,仍需继续加强小学生碘缺乏病健康教育。Objective To evaluate effects of prevention and control of iodine deficiency disorders (IDD) in Nantong city during 2003 to 2013 . Methods The goiter rate and thyromegaly of 40-50 schoolchildren aged 8-10 years in each of 30 clusters sampling by proportionate to population size (PPS) were surveyed by palpation and B-ultrasound two or three years once a time in iodine deficiency areas of Nantong city, Jiangsu Province from 2003 to 2013. The contents of household salt iodine, schoolchildren urine iodine and local representative water iodine were detected. Questionaire survey and health education of 30 students in Grade 5 in each school and 5 native women in the same village were made. Results Totally 5 174 school-age children of 8 to 10 years were surveyed in 2003,2005,2009 and 2013 in Nantong city. Goiter rates of respondents dropped from 3.26% in 2003 to 1.00% in 2013. Average rate of iodized salt coverage, qualified salt iodine and usage of qualified iodized salt were 97.5%, 95.5%, 93.2%. The content of mean salt iodine fell down slowly to 25.3mg/kg in 2013 and the medians of students urine iodine varied between 218.4μg/L to 290.8μg/L, all lower than 3001xg/L. Health education knowledge rates of pupils and women were 87.8% and 90.4%. Conclusion The stage target of ehmination of iodine deficiency disorders in Nanton'g city has been attaineded in recent years. The iodine nutrition status of schoolchildren has been maintained at an ideal level.
