摘  要:目的 观察大黄附子汤(DHFZT)对重症急性胰腺炎(SAP)大鼠肠黏膜上皮细胞(IEC)线粒体离子泵Na^+-K^+-三磷酸腺苷(ATP)酶和Ca^2+-ATP酶以及超微结构的影响.方法 72只SD大鼠,随机分成3组:对照组、SAP组(模型组)及治疗组,每组再按1、3、6、12h分为4个亚组(n=6).经胰胆管注入5%牛磺胆酸钠(4 ml/kg)建立SAP模型.造模后0、4、8h对照组和模型组用0.9% NaCl,治疗组用DHFZT保留灌肠(1ml/100 g).测定血清肠脂肪酸结合蛋白(iFABP)、二胺氧化酶(DAO)及IEC线粒体Na^+-K^+-ATP酶、Ca^2+-ATP酶水平;电镜、光镜观察胰腺、小肠结构变化.结果 经DHFZT治疗后,治疗组大鼠较模型组IEC线粒体Na^+-K^+-ATP酶[μmol Pi/mg(protein,h),1 h:6.35±0.49比6.24 ±0.46,3 h:7.01±0.57比5.45±0.39,6 h:6.57 ±0.58比4.81±0.32,P<0.05;12 h:5.98 ±0.39比4.24±0.29,P<0.01]、Ca^2+-ATP酶[μmol Pi/mg (protein·h),3 h:5.25±0.46比4.46±0.39,P<0.05;6 h:4.68±0.41比3.49±0.30,12 h:4.85 ±0.35比3.26±0.28,P<0.01]均增高,线粒体结构损伤显著减轻,血清iFABP和DAO显著降低[iFABP(μmol/L):6 h:74.67±8.57比132.46±16.43,12 h:121.48±14.35比243.43±35.91,P<0.01;DAO(g/L):3 h:100.03±16.99比178.43±26.38,P< 0.05;6 h:135.58±20.86比258.32±36.31,12 h:172.55±22.53比288.49±40.29,P<0.01].结论 DHFZT通过升高SAP时IEC线粒体离子泵Na^+-K^+-ATP酶、Ca^2+-ATP酶活性,维护IEC及其线粒体超微结构,降低血清iFABP、DAO水平.Objective To investigate the effects of Dai Huang Fu Zi Tang (DHFZT) on ultrastructure and ion pump of mitochondria in intestinal epithelial cells (IECs) of rats with severe acute pancreatitis (SAP).Methods Seventy-two SD rats were randomly divided into three groups:sham-operated group (No SAP with 0.9% NaC1 enema),model group (SAP with 0.9% NaCl enema),and treatment group (SAP with DHFZT),and each group was then subdivided into 1,3,6,and 12 h subsets (n=6each).SAP models were established by retrograde injection of 5% sodium taurocholate (4 ml/kg) into the pancreatic bile duct in model group and treatment group.Serum intestinal fatty acid binding protein (iFABP) and diamine oxidase (DAO),and ATPase in mitochondria of IECs were determined.The structural changes of mitochondria in IECs were observed under electron microscope and light microscope.Results After treatmenting with DHFZT,the levels of mitochondrial Na^+-K^+-ATPase and Ca^2+-ATPase in treatment group were significantly increased as compared with model group [for Na^+-K^+-ATPase,μ mol Pi/mg (protein·h),6.35 ±0.49 vs.6.24 ±0.46 at 1 h,7.01 ±0.57 vs.5.45 ±0.39 at 3 h,6.57±0.58 vs.4.81 ±0.32 at6 h,P〈0.05,and 5.98 ±0.39 vs.4.24±0.29 at 12 h,P〈0.01; for Ca^2+-ATPase,μmol Pi/mg (protein·h),5.25 ±0.46 vs.4.46 ±0.39 at 3 h,P〈0.05,4.68 ±0.41 vs.3.49 ±0.30 at 6 h,and 4.85 ± O.35 vs.3.26 ± 0.28 at 12 h,P 〈 0.01],damage of mitochondrial ultrastructure was also reduced,and serum iFABP and DAO were significantly decreased [for iFABP (μ mol/L):74.67 ±8.57 vs.132.46 ± 16.43 at 6 h,and 121.48 ± 14.35 vs.243.43 ±35.91 at 12 h,P 〈0.01 ; for DAO (g/L):100.03 ±16.99 vs.178.43 ±26.38 at3 h,P〈0.05; 135.58 ±20.86 vs.258.32 ±36.31at 6 h,and 172.55 ± 22.53 vs.288.49 ± 40.29 at 12 h,P 〈 0.0l].Conclusion DHFZT could protect IECs and mitochondrial normal structures and increase the levels of serum iFABP and DAO probably by regulating the levels of mitochondrial ion pum
