髓会、骨会配穴针刺治疗膝关节骨性关节炎30例临床观察及作用机制研究

摘　　要：目的研究髓会、骨会配穴针刺治疗对膝关节骨性关节炎患者血清白细胞介素-1β(IL-1β)、软骨寡聚基质蛋白(COMP)表达的影响及临床疗效,初步阐明其作用机制。方法将60例膝关节骨性关节炎患者随机分为2组,每组30例。对照组采用常规取穴针刺治疗,治疗组在对照组选穴基础上主穴增加八会穴之髓会悬钟、骨会大杼进行针刺治疗。评价2组临床疗效,比较2组治疗前后血清IL-1β,COMP水平变化及生存质量量表(SF-36)评分。结果治疗组临床痊愈率和总有效率均显著高于对照组(P<0.05);治疗后2组COMP,IL-1β水平均显著降低,SF-36评分显著升高,与同组治疗前比较差异有统计意义(P<0.05或P<0.01),且治疗组作用显著优于对照组,2组比较差异有统计意义(P<0.05或P<0.01)。结论髓会、骨会配穴针刺治疗膝关节骨性关节炎疗效显著,且方法安全,值得在临床上推广应用,其作用机制可能与降低I L-1β,COMP水平有关。Objective To observe the influence of serum IL-1β, COMP expression and the clinical curative effect of patients with knee osteoarthritis by acupuncturing at Xuanzhong （GB39） and Dazhu （UBI 1 ） as well as to clarify its mechanism. Methods 60 cases were randomly divided into treatment group and control group, 30 cases each. In control group, patients were treated with common acupuncture. In treatment group, patients were inter- vened by acupuncture at Xuanzhong （B39） and Dazhu （ UB11 ） additionally. After treatment, the clinical curative effect of both groups were evaluated. The changes of serum IL-1β, COMP and SF-36 were compared before and af- ter treatment. Results The curative rate and total effective rate of treatment group were higher than that of control group （P〈0.05） ;the levels of IL-1β and COMP reduced after treatment in both groups, particularly in the experi- mental group （P〈0.05 or P〈0.01 ）. Conclusion The treatment is evidently effective. It＇ s safe and worth popu- larizing. The mechanism is largely related to reduction of IL-1β and COMP.
