中药联合醋酸泼尼松治疗老年原发免疫性血小板减少症的疗效观察

摘　　要：目的观察中药联合醋酸泼尼松治疗老年原发免疫性血小板减少症的疗效。方法 85例老年原发免疫性血小板减少症患者随机分为两组,试验组43例采用中药联合醋酸泼尼松片治疗;对照组42例仅给予醋酸泼尼松片治疗,分别于治疗开始起1个月、6个月、1年三个时间点观察疗效。结果自治疗开始起1个月,试验组总有效率86.0%,对照组为66.7%,两组差异有统计学意义(P<0.05);自治疗开始起6个月和1年,试验组总有效率分别为72.1%和55.8%,而对照组总有效率分别为42.9%和16.7%,差异均有统计学意义(P<0.05);治疗期间,试验组患者不良反应及并发症发生率明显低于对照组患者,差异有统计学意义(P<0.05)。结论中药联合醋酸泼尼松治疗老年原发免疫性血小板减少症,可以降低糖皮质激素耐药及依赖,维持治疗的长期疗效,且能减少糖皮质激素的毒副作用,值得临床推广应用。Objective To observe the curative effect of traditional Chinese medicine combined with prednisone acetate in the treatment of senile primary immune thrombocytopenia. Methods A total of 85 patients with senile primary immune thrombocytopenia were randomly divided into two groups. The experimental group with 43 cases received traditional Chinese medicine combined with prednisone acetate for treatment, and the control group with 42 cases received prednisone acetate tablets alone for treatment. The curative effects were observed in three time points as 1 month, 6 months, and 1 year of the treatment. Results In 1 month of the treatment, the effective rate of the experimental group was 86.0%, and that of the control group was 66.7%. The difference between the two groups had statistical significance （P〈0.05）. In 6 months and 1 year of the treatment, the total effective rates of the experimental group were 72.1%and 55.8%, and they were 42.9%and 16.7%in the control group. The difference had statistical significance （P〈0.05）. During the treatment, the incidences of adverse reactions and complications were all lower in the experimental group than in the control group, and the difference was statistically significant （P〈0.05）. Conclusion The combination treatment by traditional Chinese medicine and prednisone acetate for senile primary immune thrombocytopenia can reduce drug resistance and dependence of glucocorticoid, maintain long-term effect, and decrease toxic and side effects of glucocorticoid. This method is worthy of clinical promotion and application.
