摘 要:目的 探讨护士掌握经外周静脉留置中心静脉导管(PICC)置管技术所需要的最少操作例数,为PICC置管资格准入标准的制定和技能培训提供参考依据。方法 以初学PICC置管的护士为研究对象,记录其置管过程所用时间、穿刺次数、操作评分和术后导管尖端位置四个评价指标数据并计算累积和值(CUSUM),绘制研究对象个体学习曲线图,拟合得到总体学习曲线图,曲线斜率K值在逐渐减小的过程中,开始为负值时所对应的操作次数即为掌握该项技能所需要的最少操作例数。结果 共15名研究对象操作置管共计225例,个体学习曲线斜率开始为负值的操作例数为3-14例,总体学习曲线自第8例操作起开始为负值。结论 掌握PICC置管技能所需要的最少操作例数是8例。Objective To study the least practice number for nurses to gain the skill of insertion of peripherally inserted central venous catheter(PICC), and to provide references for future training program and qualification standard system. Method Nurses received PICC training was included in this study. Total time of insertion, attempt numbers of puncture, practice score and location of catheter tip were recorded prospectively as evaluating indicator, and cumulated the sum of the Cumulative Summation(CUSUM). Established a individual learning curve and total learning curve. The minimum training cases to master. This skill is assessed by the number of practices corresponding to the curve slope turn into negative. Result 15 nurses were included in this study, total 225 catheter cases were finished. Statistical analysis showed that the number corresponding to the changement of slope changed into negative was 3th-14th case among individual learning curves, which was confirmed to 8th case in the total learning curve. Conclusion At least, 8 cases for PICC catheter practice is needed to master this skill.
