摘 要:目的:探讨老年2型糖尿病(T2DM)患者糖化血红蛋白(HbAlc)控制水平与脂蛋白相关磷脂酶A2(Lp—PLA2)浓度和肾功能指标变化的临床意义。方法:选择2013年8月-2014年2月间在我院内分泌科和心内科住院已经确诊的老年2型糖尿病患者221例,根据患者HbAlc水平不同分为3组:A组:HbAlc〈7.0%(n=112),B组:HbAlc7.0~9.0%(n=64),C组:HbAlc〉9.0%(n=45),检测其Lp—PLA2、超敏-CRP(hs—C1LP)和肾功能指标f胱抑素(CysC)、尿素(UREA)、肌酐(cr)1,并对检测结果进行统计学处理及比较分析。结果:A组患者血清Lp—PLA2含量明显低于B组和c组,差异具有统计学意义(P〈0.001),B组与c组比较差异无统计学意义(P〉0.05);A组CysC、UREA、Cr异常率明显低于c组,差异具有统计学意义(P〈o.05),B组与c组比较差异无统计学意义(P〉o.05);C组hs—CRP异常率显著高于A组和B组,差异具有统计学意义(P〈0.05),A组与B组比较差异无统计学意义(P〉0.05)。结论:老年T2DM患者Lp—PLA2和hs—CRP水平升高及肾功能损伤与血糖控制不良有密切的关系,定期检测HbAlc、Lp—PLA2和肾功能指标,了解这些指标的变化情况,对患者血糖水平的监控及其对并发症的监测和疗效观察有较高的价值。Objective : To investigate the clinical significance of concentration changes of lipoprotein associated phospholipase A2, renal function and HbAlc in elderly patients with type 2 diabetes. Methods : 221 patients with type 2 diabetes were chosen in Department of Endocrinology and Department of Cardiology in our hospital from August 2013 to February2014. They were divided into 3 groups according to the HbAlc levels: group A: HbAlc〈7.0% (n=112), group B: HbAlc7.0 ~ 9.0%(n=64), group C: HbAlc〉9.0% (n=45), Lp- PLA2, high sensitive -CR.P (hs-CRP) and renal function indices [cystatin (CysC), urea (UREA), creatinine (Cr)] were detected, and the testing results were comparative and statistical by analyzed.Results : Serum Lp-PLA2 in group A was significantly lower than that in group B and group C, the difference was statistically significant (P〈0.001), but there was no significant difference between group B and group C (P〉0.05). The abnormal rates of CysC, UREA and Cr in groupA were significantly lower than that in groupC,the difference was statistically significant P〈0.05), but there was no significant difference between the group B and groupC (P〉0.05); The abnormal rate of hs- CRPin group C was significantly higher than that in groupA and groupB, the difference was statistically significant P〈0.05), but there was no significant difference between the group A and groupB (P〉0.05). Conclusions : High levels of Lp-PLA2 and hs-CRP and renal damage are closely related to poor glycemic control. Regular detection of HbAlc, Lp-PLA2 and renal function indices and understanding the changes of these indices have high value on monitoring of blood glucose level and observation of complications.
