经脐单孔双通道腹腔镜下丝线法精索静脉高位结扎术(附65例报告)

摘　　要：目的：探讨经脐单孔双通道腹腔镜下丝线法精索静脉高位结扎术的临床疗效与手术技巧。方法：回顾分析原发性精索静脉曲张65例患者的临床资料。患者16~65岁,平均（22.0±3.8）岁。入组标准为中重度精索静脉曲张、阴囊坠胀不适或疼痛症状明显、不育,排除继发性病因。行经脐单孔双通道腹腔镜下丝线法精索静脉高位结扎术。结果：65例手术均顺利完成,手术时间15~45 min,平均（20.0±3.5）min;术中出血量1~10 ml,平均（3.0±1.2）ml;无严重并发症发生。术后患者均获随访,随访3~15个月,临床症状缓解或消失,精液质量明显提高。无睾丸萎缩、术后复发。结论：经脐单孔双通道腹腔镜下丝线法精索静脉高位结扎术安全、有效、美观、费用低。Objective：To explore the effect and skills of transumbilical dual-channel single-port laparoscopic varicocelectomy with silk thread. Methods ：The clinical data of 65 varicocele patients were retrospectively analyzed. They were 16 to 65 years old, with the average of （22.0 ± 3.8 ）. The inclusion criteria were moderate and serious varicocele, scrotum bulge or obvious pain, infertility, without any secondary causes. All patients were treated with transumbilical dual-channel single-port laparoscopic varicocelectomy. Re- suits：All the operations were completed successfully without any serious complications. The operation time was 15-45 min, with an aver- age of （20.0± 3.5 ） min. The mean intraoperative blood loss was （3.0 ± 1.2） ml （range, 1-10 ml）. All patients were followed up for 3-15 months. Their clinical symptoms got relieved or disappeared, the semen quality obviously improved without testieular atrophy or re- currence. Conclusions：Transumbilical dual-channel single-port laparoscopic varicocelectomy with silk thread is safe and efficient, with satisfying cosmetic result and low cost.
