复位外固定器治疗胸腰椎骨折的影像学评价

摘　　要：目的:从影像学角度评估脊柱复位外固定器在治疗胸腰椎压缩性骨折中的价值。方法:治疗组30例胸腰椎压缩性骨折患者采用手法复位后新型可调脊柱外固定夹板治疗,对照组30例予垫枕治疗,治疗前、后行X线摄片及CT检查,观察比较两组患者治疗前后不同时间伤椎椎体高度百分比、Cobb角的变化。结果:治疗组患者治疗后椎体高度、后凸Cobb角等较术前均有显著恢复(P<0.05),较对照组均改善明显(P<0.05),治疗后当天和治疗后8周影像学观察指标差异无统计学意义(P>0.05)。结论:可调脊柱外固定夹板治疗胸腰椎压缩性骨折可有效恢复和维持压缩椎体高度,疗效显著,并且操作简便,具有创伤小、恢复快、费用低廉等优点。Objective：To evaluate the effect of adjustable spinal external fixation in the treatment of thoracolumbar fracture from an imageological point of view.Methods：30 patients of thoracolumbar fracture were treated by manual reduction and the new-style adjustable spinal external fixation; and 30 other patients from the control group were treated with pillow.Prior to and after the treatment,the subjects were examined by X-ray and CT so we could compare the height of injured column and Cobb＇s angle between the two groups.Results：Inside the experimental group,all the patients＇ column height and cobb＇s angle had significantly （P ＜ 0.05） improved compared to those from the control group （P ＜ 0.05）.There was no significant difference between the two groups immediately after the treatment and 8 weeks later in observation indicators of imageology（P ＞0.05）.Conclusion：Adjustable spinal external fixation has significant curative power in treating thoracolumbar fracture in terms of restoring or maintaining the height of the injured column.In addition,this treatment is less expesive and easier to operate,produces smalled damages,and the patients also recover faster.
