人工全膝置换术后隐性失血的临床相关性分析

摘　　要：目的探讨人工全膝置换术(total knee arthroplasty,TKA)后隐性失血的特点及其临床影响因素。方法 2005年5月-2013年8月已在本院行初次单侧TKA术患者中随机抽取78例并通过Gross方程计算隐性失血量,最后分析患者性别、年龄、体重指数(body mass index,BMI)、止血带时间、手术时间、术前诊断与隐性失血的相关性。结果总失血量(1307.54±1431.342)ml,术中出血量(338.46±276.667)ml,术后出血量为(374.43±282.531)ml,隐性失血量(594.65±872.144)ml,隐性失血量占总失血量的45.5%,其中输血患者高达62.5%。结论隐性失血量与术中出血量、术后出血量及年龄呈明显负相关性,与手术时间呈明显正相关。Objective To explore the hidden blood in total knee arthroplasty （TKA） and determine the clinical factors affecting the hidden blood loss. Methods We randomly selected 78 patients who have underwented primary unilateral TKA in our hospital from May 2005 to August 2013. The hidden blood loss was calculated through Gross formula. It was analyzed that whether the hidden blood following TKA were affected by sex,age,BMI,tourniquet time,operation time and diagnose. Results The total loss was （1307.54±1431.342）mL,intral-operative blood loss was （338.46±276.667）ml,post-operative blood loss was （374.43±282.531） ml,hidden blood loss was （594.65±872.144）ml,the hidden blood accounts for 45.5%of total blood loss,and the proportion of blood transfusion was up to 62.5%. Conclusion Hidden blood loss was negative corretated with the intral-operative blood loss ,post-op-erative blood loss and age,but it was positively correlated with operation time.
