芪苈强心胶囊对老年高血压并心力衰竭患者血浆心肌营养素-1的影响

摘　　要：目的:探讨芪苈强心胶囊对老年高血压并心力衰竭(心衰)患者血浆心肌营养素-1(CT-1)、N末端B型利钠肽原(NT-proBNP)水平的影响。方法:选择年龄≥65岁的高血压并心衰患者80例,随机分为治疗组42例和对照组38例,对照组常规抗心衰治疗,治疗组常规抗心衰治疗加用芪苈强心胶囊,4周后测定血浆CT-1、NT-proBNP和左心室射血分数(LVEF)。血浆CT-1水平采用双抗体夹心酶联免疫吸附法(ELISA)测定,NTproBNP水平采用锐普荧光干式定量分析仪检测,GE Vivid 7超声心动图测定LVEF。结果:治疗后两组血浆CT-1及NT-proBNP水平均较治疗前降低(均P<0.01);治疗组血浆CT-1、NT-proBNP水平及地高辛、呋塞米用量较对照组下降更明显(均P<0.05),LVEF较对照组显著提高(P<0.01)。治疗组总有效率较对照组高(90.48%︰73.68%,P<0.05)。结论:芪苈强心胶囊通过降低老年高血压并心衰患者血浆CT-1和NT-proBNP水平,减少强心剂和利尿剂用量,改善心功能。Objective：To observe the changes of plasma cardiotrophin-l（CT-1） and N terminal probrain natri- uretic （NT-proBNP） in elderly hypertensive patients with heart failure by Qili Qiangxin Capsule therapy. Method： Eighty hypertensive patients with heart failure, aged 65 or over were randomly divided into treatment group. （n= 42） and control group （n ： 38）. The control group received routine anti-heart failure treatment. The treatment group received conventional therapy plus Qili Qiangxin Capsule for 4 weeks. Plasma CT-1, NT-proBNP and left ventricular ejection （LVEF） were determined in all subjects. Plasma CT-1 level was measured by a double anti- body sandwich enzyme-linked immunosorbent assay （ELISA）. NT-proBNP concentration was tested by Pu Rui fluorescent dry quantitative analyzer. LVEF was evaluated by GE Vivid 7 Doppler echocardiography. Result： Plas- ma CT-1 and NT-proBNP of two groups were lower after treatment （both P〈0.01）. The levels of plasma CT-1 and NT-prol3NP in treatment group were significantly decreased than those in control group （both P〈0.05）. Compared with control group, lower dosage of digoxin and furosemide were used in treatment group （P〈0.05）. The improvement of LVEF was better in treatment group than in control group （P〈0.01）. There was statistic difference in total effective rate （ 90.48% ： 73.68%, P〈0.05）. Conclusion： Conventional therapy plus Qili Qiangxin Capsule can decrease plasma CT-1 and NT-proBNP levels in elderly hypertensive patients with heart fail- ure, reduce the dosage of digoxin and furosemide and improve heart function.
