闭合复位空心钉和解剖型跟骨钛板内固定治疗SandersⅡ、Ⅲ型跟骨骨折疗效比较

摘　　要：目的比较闭合复位空心钉和解剖型跟骨钛板内固定治疗SandersⅡ、Ⅲ型跟骨骨折的临床疗效。方法选取自2010-01—2012-10诊治SandersⅡ、Ⅲ型跟骨骨折60例（64足）,采用闭合复位空心钉内固定（A组）和解剖型跟骨钛板内固定（B组）治疗各30例（32足）。比较2组手术时间、术中出血量、术后并发症,术后Bohler角、Gissane角、Kofoed评分、Maryland评分。结果 60例均获得随访12~16个月,平均13.2个月。2组手术时间、术中出血量和术后早期并发症发生率差异有统计学意义（P〈0.05）,A组明显优于B组;2组Maryland评分、Kofoed评分、Bohler角、Gissane角、晚期并发症发生率差异无统计学意义（P〉0.05）。结论对于SandersⅡ、Ⅲ型跟骨骨折的治疗,闭合复位空心钉和解剖型钛板内固定均能取得良好的临床疗效,但前者相较后者更有优势,可作为优先选择的治疗方法。Objective To compare the clinical outcomes between closed reduction hollow screw fixation and anatomic calcaneal titanie plate fixation for the treatment of Sanders type Ⅱ,Ⅲ caleaneal fracture. Methods From January 2010 to October 2012, 60 patients（64 feet） suffered from Sanders type Ⅱ,Ⅲ caleaneal fracture served as the object of the study. Among them, 30 patients （32 feet） were surgically treated by closed reduetion hollow screw fixation as group A, while the other 30 patients （32 feet） were treated with anatomic calcaneal titanic plate fixation as group B. The operation time, blood loss, eomplieation rate, the Bohler angle and Gissane angle after operation, Maryland Foot Score and Kofoed Foot Seore were compared by the statistieal analysis. Results All patients were followed up for 13.2 months averagely （12-16 months）. In operation time, blood loss and early complication rate there were significant differences （P 〈0.05） between the two groups, group A was better than group B. In Maryland Foot Score, Kofoed Foot Score, Bohler angle, Gissane angle and long-term complieation rate there were also no significant differences （P 〉0.05） between the two groups. Coneluslon As for treating Sanders type Ⅱ,Ⅲ calcaneal fracture, both elosed reduction hollow screw fixation and anatomic calcaneal titanic plate fixation can get a better clinical outcome, but the former has the advantages in the treatment of Sanders type Ⅱ,Ⅲ calcaneal fractures and can be as a priority treatment.
