摘 要:目的探讨2型糖尿病患者CA199的变化特点及相关影响因素。方法收集180例住院2型糖尿病患者,并按Hb AlC水平分为血糖控制差组(A组,56例,Hb A1C〉7.5%)、控制尚可组(B组,59例,Hb A1C 6.5%~7.5%)、控制良好组(C组,65例,Hb A1C〈6.5%),选取同期60例健康人群作为正常对照组(D组)。检测各组血清CA199、空腹血糖(FBG)、糖化血红蛋白(Hb A1C)、空腹胰岛素(FINS)、空腹C肽(FCP)年龄、性别、体质量指数(BMI)、腰围、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇等指标。比较各组CA199的水平,并分析与上述各变量间的相关性。以ln CA199为因变量,其余指标为自变量,进行多元线性回归分析,比较各变量对其影响程度。结果方差分析显示A、B组CA199显著高于C、D组,且A组更高于B组;Spearman相关分析显示CA199水平与FBG(r=0.179,P=0.016)、Hb A1C(r=0.424,P〈0.001)呈正相关;多元线性回归分析显示:Hb A1C是影响ln CA199的独立危险因素(β=0.388,P〈0.001)。结论血清CA199在2型糖尿病患者中有低水平升高,考虑可能与血糖长期控制不佳密切相关。Objective To investigate the variance of serum CA199 levels and its related factors in type 2 diabetic patients. Methods One hundred and eighty hospitalized type 2 diabetic patients were enrolled in the study and were divided into3 groups accordingtoHb A1 c levels: group A( Hb A1 C 7. 5%,n = 56),group B( Hb A1 C 6. 5%-7. 5%,n = 59) and group C( Hb A1 C 6. 5%,n = 65);and 60 healthy subjects were alsoenrolled as normal controls( group D). The levels of serum CA199,fastingblood glucose,glycosylated hemoglobin,fastinginsulin,fastingC-peptide,age,sex,body mass index,waist circumference,total cholesterol,triglyceride,high density lipoprotein cholesterol,lowdensity lipoprotein cholesterol and other variables were used for analysis. The levels of serum CAl99 were compared amongthe 4 groups,and the correlation of CA199 with other variables was analyzed with multiple linear regression. Results CA199 levels in groups A and B were significantly higher than groups C and D,and that of group A was the highest. Spearman relation analysis showed that the CAl99 level was positively correlated with FBG( r = 0. 179,P = 0. 016),Hb A1C( r = 0. 424,P〈0. 001). M ultiple linear regression analysis showed that Hb AlC was the independent risk factor for increased CAl9( β = 0. 388,P〈0. 001). Conclusion The low-level elevation of serum CA199 in type2 diabetic patients may be closely related tolong-term poor glucose control.
