摘 要:目的探讨超声造影结合参数图像处理在卵巢肿瘤的诊断及良恶性鉴别中的应用价值。方法随机选取50例卵巢肿瘤的患者,根据其后期临床病理诊断结果分为良性肿瘤组与恶性肿瘤组。术前进行超声造影检查,记录其超声造影检查结果以及各病例时间—强度曲线(TIC曲线),并结合参数图像处理定量分析TIC曲线各指标,最后判断超声造影结合参数图像处理的应用价值。结果 50例患者与术后病理检查结果相比较,常规超声的良性肿瘤检出率为51.85%,恶性肿瘤检出率为39.13%;超声造影结合参数图像处理的良性符合率为77.78%,恶性符合率为73.91%,良恶性检出率比较差异有统计学意义。良恶性肿瘤造影的图像有差异,其定量分析良恶性肿瘤TIC曲线的指标:达峰强度(Imax)、始增时间(RT)、达峰时间(TTP)、平均渡越时间(m TT)差异均有统计学意义(均P<0.05)。结论超声造影检查能较好显示卵巢良恶性肿瘤之间的差异,结合参数图像处理软件能进一步提高良恶性肿瘤的诊断及鉴别诊断价值。Objective To investigate the application value of contrast-enhanced ultrasonography combined with parametric image processing in the diagnosis and differential diagnosis of overian tumor. Methods Fifty patients with overian tumor were divided into benign tumor group and malignant tumor group according to pathological diagnosis.All patients were proceeded by conventional ultrasonography and contrast-enhanced ultrasonography.The results of contrast-enhanced ultrasonography and time-intensity curve(TIC)were recorded.TIC indexes were analyzed quantitatively using parametric image processing.Results Compared with pathological results,detection rate of benign tumor using conventional ultrasonography was51.85%,and detection rate of malignant tumor was 39.13%.The coincidence rate of benign tumor using contrast-enhanced ultrasonography combined with parametric image processing was 77.78%,and coincidence rate of malignant tumor was 73.91%.There were significant differences in benign and malignant detection rates between the two methods.There were significant differences in Imax,RT,TTP and m TT between benign and malignant groups(P 〈 0.05).Conclusion Contrast-enhanced ultrasonography can show the difference between benign and malignant ovarian tumor.Contrast-enhanced ultrasonography combined with parametric image processing can improve further the value of diagnosis and differential diagnosis in benign and malignant ovarian tumor.
